escription
The preparation "Brionit" is used to reduce intraocular pressure (IOP) in patients with chronic open-angle glaucoma and intraocular hypertension with insufficient effectiveness of local beta-blockers.
Structure
1 ml of solution contains (active ingredients):
brimonidine tartrate - 2.0 mg;
timolol maleate - 6.8 mg (in terms of timolol - 5.0 mg).
Excipients: benzalkonium chloride; sodium dihydrogen phosphate, monohydrate; sodium hydrogen phosphate, heptahydrate; diluted hydrochloric acid; sodium hydroxide; water for injections.
Contraindications
hypersensitivity to any component of the preparation;
increased airway reactivity, including bronchial asthma and cases of bronchial obstruction, including a history of severe chronic obstructive pulmonary disease;
sinus bradycardia, sinus node dysfunction syndrome, atrioventricular block, II-III degree blockade without an implanted artificial pacemaker, heart failure, cardiogenic shock;
concomitant therapy with monoamine oxidase (MAO) inhibitors, antidepressants that affect noradrenergic transmission (tricyclic antidepressants and mianserin).
Mode of application
The recommended dose for adults, including elderly patients: one drop of Brionit should be instilled into the conjunctival sac of the affected eye 2 times a day with an interval of 12 hours. If you use 2 or more ophthalmic preparations, you must take a 5-minute break between instillations.
As with other eye drops, to reduce possible systemic absorption, it is recommended to briefly press on the lacrimal sac in the place of its projection at the inner corner of the eye or close the eyelids for two minutes.
This should be done immediately after instillation of each drop in order to reduce systemic side effects and enhance local action.
Application features
Pregnant
Controlled studies on the use of brimonidine with timolol in pregnant women have not been carried out, therefore, the use of the preparation during pregnancy is contraindicated.
Timolol passes into breast milk, therefore, the use of the preparation "Brionit" during breastfeeding is contraindicated.
Children
The safety and efficacy of using the preparation "Brionit" for children have not been established, therefore it cannot be used in pediatric practice.
Drivers
Brimonidine with timolol has little effect on the ability to drive vehicles and mechanisms. Against the background of the use of the preparation"Brionit", a short-term visual impairment (blurring), the development of weakness and drowsiness, which worsen the speed of reactions, is possible. In the event of the occurrence of these symptoms, one should refrain from activities requiring special attention.
Overdose
Overdose of brimonidine with timolol in humans has been rarely reported, which did not lead to adverse reactions. Overdose treatment includes supportive and symptomatic therapy. The patient's breathing should be maintained.
Side effects
The most common side effects were conjunctival hyperemia (approximately 15% of patients) and a burning sensation of the mucous membrane of the eye (approximately 11% of patients) based on data from a 12-month clinical study. In most cases, the severity of these symptoms was mild; withdrawal of therapy was necessary only in 3.4% and 0.5% of cases, respectively.
