    Pharmacological properties

    Pharmacodynamics. The mechanism of the antimycotic action of clotrimazole is associated with the suppression of the synthesis of ergosterol, which leads to structural and functional damage to the cytoplasmic membrane.
    Clotrimazole has a wide spectrum of antimycotic activity in vitro and in vivo, acts on dermatophytes, yeast, mold and dimorphic fungi.
    Under appropriate testing conditions, the minimum inhibitory concentrations for these types of fungi are approximately 0.062–8.0 μg / ml of substrate.
    The mechanism of action of clotrimazole is associated with primary fungistatic or fungicidal activity, depending on the concentration of clotrimazole at the site of infection.
    In vitro activity is limited to the proliferating elements of the fungus; spores of the fungus have only a slight sensitivity.
    In addition to the antimycotic activity, clotrimazole also acts on gram-positive microorganisms (streptococci, staphylococci, Gardnerella vaginalis) and gram-negative microorganisms (Bacteroids)).
    In vitro, clotrimazole inhibits the growth of Corynebacteria and gram-positive cocci (with the exception of Enterococci) at a concentration of 0.5–10 μg / ml of substrate. Initially resistant strains of susceptible fungal species are rare. The development of secondary resistance during treatment in susceptible fungi has been very rare so far.
    Preclinical studies conducted with the participation of volunteers using a single and repeated toxic dose did not reveal any harmful toxic, genotoxic effects and effects on reproduction.
    Pharmacokinetics. Pharmacokinetic studies after vaginal administration have shown that only a small amount of clotrimazole (3–10%) is absorbed. Absorbed clotrimazole is rapidly metabolized in the liver to inactive metabolites. Therefore, the plasma Cmax of clotrimazole after vaginal administration at a dose of 500 mg was 10 ng / ml. This means that the occurrence of measurable systemic effects or side effects after intravaginal use of clotrimazole is unlikely.

    Indications

    Infections in the genital area (vaginitis) caused by fungi (usually of the genus candida) and superinfections caused by bacteria susceptible to clotrimazole.

    Application

    Vaginal suppository 100 mg
    The vaginal suppository should be inserted into the vagina as deeply as possible before going to bed. It is more convenient to administer the suppository lying on your back with slightly bent legs.
    One course of therapy for 2 suppositories for three days or 1 suppository for six days.
    A second course of therapy may be prescribed if needed.
    Treatment should not be carried out during menstruation. Treatment should be completed before the onset of menstruation.
    Do not use tampons, intravaginal irrigations, spermicides, or other vaginal products while using this preparation.
    Sexual contact should be avoided while using this preparation , as the infection can be passed on to your partner.

    Contraindications

    Hypersensitivity to clotrimazole or other components of the preparation.

    Side effects

    From the immune system: allergic reactions, including fainting, arterial hypotension, shortness of breath, urticaria, itching.
    From the reproductive system and mammary glands: skin peeling in the genital area, itching, rash, edema, erythema, discomfort, burning, irritation, pelvic pain, vaginal bleeding.
    From the gastrointestinal tract: abdominal pain.

    Special instructions

    Contact with eyes should be avoided. do not swallow.
    The patient is advised to consult a doctor:
    at the first manifestations of the disease;
    if symptoms persist for 7 days;
    if symptoms appear again within 2 months or more than 4 times during the last 12 months;
    with established hypersensitivity to any antifungal agents or imidazole derivatives;
    in the presence of a history of sexually transmitted diseases or a burdened history of sexually transmitted diseases in a partner;
    if you have more than two episodes of candidal vaginitis in the last 6 months;
    if the patient has a high body temperature (≥38 ° C), pain in the lower abdomen, dysuria, back pain, purulent vaginal discharge with an unpleasant odor, vulvar and vaginal ulcers, redness, diarrhea, nausea, vomiting, vaginal bleeding, accompanied by pain in the shoulder.
    During the use of the preparation, sexual intercourse should be avoided, since the infection can be transmitted to the partner. The effectiveness and safety of latex products (such as condoms and diaphragms) may decrease (see INTERACTIONS).
    Since both the vagina and the external genitalia (labia and adjacent areas) are commonly affected, a combination treatment of both areas should be used, including topical topical cream treatment.
    During treatment with Clofan, vaginal suppositories, it is recommended to treat both partners with a cream.

    Use during pregnancy and lactation. Clinical studies of the effect of clotrimazole on fertility in women have not been carried out, however, animal studies have not revealed the effect of clotrimazole on fertility.
    Animal studies have not shown any direct or indirect harmful effects in terms of reproductive toxicity. The number of studies with the use of clotrimazole during pregnancy is limited, therefore it is recommended to avoid the use of the preparation Clofan, vaginal suppositories, during the first trimester of pregnancy.
    During the period of treatment with Clofan, vaginal suppositories, breastfeeding should be stopped.
    Children. Do not use in children under 12 years of age. In children over 12 years of age, use after consulting a doctor.
    The ability to influence the reaction rate when driving or working with other mechanisms.
    The preparation does not affect or has a slight effect on the ability to drive a vehicle or work with other mechanisms.

    Interactions

    Concomitant treatment with vaginal clotrimazole and oral tacrolimus (fk-506; immunosuppressant) or sirolimus may increase plasma levels of tacrolimus / sirolimus. therefore, the patient's condition should be carefully monitored to detect symptoms of tacrolimus or sirolimus overdose, if necessary, by checking their blood plasma levels.
    The simultaneous use of vaginal suppositories with latex contraceptives (such as a condom and diaphragm) can damage the latter, so the effectiveness of these contraceptives may be reduced. The specified effect is temporary and can occur only during the treatment period. Patients are advised to use alternative methods of contraception for at least 5 days after using this preparation.

    Overdose

    There is no risk of acute intoxication, since it is unlikely that overdose is possible after a single vaginal dose or application to the skin (use on large areas of the skin under conditions conducive to increased absorption), as well as after accidental oral administration. there is no specific antidote.
    However, in case of accidental oral use and only when clinical symptoms of an overdose (such as dizziness, nausea or vomiting) appear, preventive measures such as gastric lavage should be used. Gastric lavage should only be performed when adequate respiratory protection is available.
