Pharmacological properties
Pharmacodynamics. Clotrimazole is a topical antifungal preparation from the group of imidazole derivatives. the preparation has a wide range of antimycotic activity against dermatophytes, yeasts and molds. it is also active against trichomonas, staphylococci, streptococci and bacteroids. does not affect lactobacilli.
The mechanism of the antimycotic action of clotrimazole occurs by inhibiting the synthesis of ergosterol, which leads to structural and functional damage to the cytoplasmic membrane. Depending on the concentration of clotrimazole at the site of infection, the preparation has fungistatic or fungicidal activity. In vitro, clotrimazole exhibits antifungal activity mainly against proliferating fungal elements, since fungal spores have only a slight sensitivity to the preparation.
Among fungi susceptible to clotrimazole, primary resistant strains are extremely rare. The development of secondary resistance in clotrimazole-sensitive fungal strains has been very rarely detected so far.
Pharmacokinetics. Pharmacokinetic studies after vaginal administration have shown that only a small amount of clotrimazole (3-10%) is absorbed. Absorbed clotrimazole is rapidly metabolized in the liver to inactive metabolites. Therefore, the peak plasma concentration of clotrimazole after vaginal administration at a dose of 500 mg was 10 ng / ml. This means that the occurrence of systemic effects or side effects after intravaginal use of clotrimazole is unlikely.
Indications
Infections in the genital area (vulvovaginitis) caused by fungi (usually of the genus candida) or bacteria sensitive to clotrimazole.
Application
The cream must be injected intravaginally using the supplied applicator.
7 g of Clofan cream (1 full applicator) is injected as deep as possible into the vagina in the evening (before bedtime) once. The course of treatment is 1 day. Treatment should not be carried out during menstruation, therefore, the use of the preparation must be completed before it begins. Therapy for these diseases requires the simultaneous treatment of both partners.
Contraindications
Hypersensitivity to clotrimazole or other components of the preparation.
Side effects
Skin and subcutaneous tissue disorders: skin rash, itching, erythema, edema, urticaria, irritation, peeling, fever, burning sensation.
From the reproductive system and mammary glands: allergic reactions from the vaginal mucosa, peeling of the skin in the genital area, itching, rash, swelling, discomfort, burning, irritation, pelvic pain.
From the gastrointestinal tract: abdominal pain.
General disorders and reactions at the injection site: fainting, arterial hypotension, shortness of breath.
Special instructions
Sexual contact should be avoided while using this preparation, as the infection can be passed on to your partner. treatment of both sexual partners with clofan cream is recommended. during menstruation, treatment should not be carried out. it should be finished before the onset of menstruation. do not use tampons, intravaginal irrigation, spermicides, or other vaginal preparations while this preparation is being used.
Use during pregnancy and lactation. According to the results of studies with the participation of pregnant women, there are no epidemiological data indicating the expected harmful effects on the woman and the fetus when using clotrimazole during pregnancy. During pregnancy and lactation, the use of clotrimazole is possible only if, in the opinion of the doctor, the benefit to the mother outweighs the risk to the fetus / child.
During pregnancy, another dosage form of the preparation should be used without the use of an applicator.
Children. There is no data on the safe use of the preparation in children under the age of 16, so the preparation Clofan should not be used in people under the age of 16. Children over 16 years of age - apply after consulting a doctor.

Interactions
Concomitant treatment with vaginal clotrimazole and oral tacrolimus (fk-506; immunosuppressant) may increase plasma levels of tacrolimus. therefore, the patient's condition should be carefully monitored to detect symptoms of tacrolimus overdose, if necessary, by monitoring its blood plasma level.
With the simultaneous use of clotrimazole with nystatin, the activity of clotrimazole may decrease. When administered vaginally, the preparation reduces the activity of amphotericin B and other polyene antibiotics.
The simultaneous use of a cream in the genital area with latex contraceptives (such as a condom and diaphragm) can damage the latter, so the effectiveness of these contraceptives may be reduced. This effect is temporary and can only occur during treatment. Patients are advised to use alternative methods of contraception for at least 5 days after using this preparation.
Overdose
Overdose with intravaginal use is practically not noted, given the very low absorption of the preparation.
In case of accidental ingestion, only when clinical symptoms of an overdose (such as dizziness, nausea or vomiting) appear, preventive measures such as gastric lavage should be used.
