Pharmacological properties
Corvaltab is a sedative, vasodilator, antispasmodic agent. helps to reduce the excitability of the central nervous system, has a calming effect and facilitates the onset of natural sleep. ethyl ester of α-bromisovaleric acid, which is part of the preparation Corvaltab, has a sedative and antispasmodic effect, similar to the action of valerian extract; in high doses, it also causes a mild hypnotic effect. phenobarbital has a sedative and moderate antispasmodic effect. peppermint oil causes reflex vasodilating and antispasmodic effects.
Indications
Neuroses with increased irritability;
insomnia;
in the complex therapy of hypertension and vascular dystonia;
mild spasms of the coronary vessels, tachycardia;
intestinal spasms due to neurovegetative disorders (as an antispasmodic preparation).
Application
The dose and duration of the course of treatment are set by the doctor individually for each patient.
Adults are usually prescribed 1-2 tablets of the preparation 2-3 times a day before meals. With tachycardia and spasms of the coronary vessels, a single dose can be increased to 3 tablets.
Contraindications
Hypersensitivity to the components of the preparation, severe liver and / or kidney dysfunction, hepatic porphyria, severe heart failure.
Medicines containing phenobarbital are contraindicated in severe arterial hypotension, acute myocardial infarction, diabetes mellitus, depression, myasthenia gravis, alcoholism, preparation and preparation addiction, respiratory diseases with shortness of breath, obstructive syndrome.
Side effects
Corvaltab is generally well tolerated.
In some cases, the following side effects may be noted:
from the digestive system: constipation, a feeling of heaviness in the epigastric region, with prolonged use - impaired liver function, nausea, vomiting;
from the nervous system: weakness, ataxia, impaired coordination of movements, nystagmus, hallucinations, paradoxical agitation, decreased concentration, fatigue, slow reaction rate, headache, cognitive impairment, confusion, drowsiness, light dizziness;
on the part of the hematopoietic organs: anemia, thrombocytopenia, agranulocytosis;
on the part of the cardiovascular system: arterial hypotension, bradycardia;
from the immune system: hypersensitivity reactions, including angioedema, allergic reactions (including skin rash, itching, urticaria);
on the part of the skin and mucous membranes: Stevens-Johnson syndrome, toxic epidermal necrolysis;
on the part of the musculoskeletal system: with prolonged use of preparations containing phenobarbital, there is a risk of impaired osteogenesis;
other: shortness of breath.
Long-term use of preparations containing bromine can lead to bromine poisoning, which is characterized by the following symptoms: depression of the central nervous system, depressed mood, confusion, ataxia, apathy, conjunctivitis, rhinitis, lacrimation, acne, or purpura.
They pass when the dose is reduced or the preparation is stopped.
Special instructions
During treatment with the preparation, it is not recommended to engage in activities requiring increased attention, speed of mental and motor reactions.
Drinking alcohol should be avoided while taking the preparation.
The presence of phenobarbital in the composition of the preparation can lead to the risk of developing Stevens-Johnson and Lyell syndrome, which is most likely in the first weeks of treatment. Long-term use is not recommended due to the risk of preparation dependence, possible accumulation of bromine in the body and the development of bromine poisoning. In cases where pain in the region of the heart does not disappear after taking the preparation, it is necessary to consult a doctor to exclude acute coronary syndrome. Care should be taken to prescribe the preparation for arterial hypotension, hyperkinesis, hyperthyroidism, adrenal hypofunction, acute and persistent pain, acute preparation intoxication.
The preparation contains lactose, therefore Corvaltab is not recommended for patients with hereditary lactose-galactose intolerance.
Use during pregnancy and lactation. The preparation is not prescribed during pregnancy and lactation.
Children. There is no experience in the treatment of children, so the preparation is not used in pediatric practice.
The ability to influence the reaction rate when driving or operating machinery. Corvaltab can cause drowsiness and dizziness, therefore, during the period of treatment, patients are not recommended to work with dangerous mechanisms and drive vehicles.
Interactions
With the simultaneous use of the preparation with other preparations that depress the central nervous system, mutual enhancement of the action is possible. the effect of the preparation is enhanced by the use of preparations valproic acid, alcohol. phenobarbital induces liver enzymes and, accordingly, can accelerate the metabolism of some preparations that are metabolized by liver enzymes (including indirect anticoagulants, cardiac glycosides, antimicrobial, antiviral, antifungal, antiepileptic, anticonvulsant, psychotropic, oral hypoglycemic, immunosuppressive, hormonal antihypertensive preparations, etc.).
Phenobarbital enhances the action of analgesics and local anesthetics.
MAO inhibitors prolong the effect of phenobarbital. Rifampicin may reduce the effect of phenobarbital.
With the simultaneous use of phenobarbital with gold preparations, the risk of kidney damage increases.
With prolonged simultaneous use of phenobarbital with NSAIDs, there is a risk of developing stomach ulcers and bleeding.
The simultaneous use of phenobarbital with zidovudine increases the toxicity of both preparations.
The preparation increases the toxicity of methotrexate.
Overdose
It is possible with frequent or prolonged use of the preparation, which is associated with the cumulation of its components. long-term and constant use can lead to addiction, withdrawal symptoms, psychomotor agitation.
Symptoms: respiratory depression, even to the point of stopping it; depression of the central nervous system, up to coma; suppression of cardiovascular activity, including rhythm disturbances, lowering blood pressure, up to a collaptoid state; nausea, weakness, decreased body temperature, decreased urine output.
Treatment: symptomatic.
