Clindamycin is a lincosamide antibiotic that inhibits bacterial protein synthesis by acting on bacterial ribosomes. The antibiotic binds predominantly to the 50S ribosomal subunit and affects the translation process. Although clindamycin phosphate is inactive in vitro, in vivo it is rapidly hydrolyzed to clindamycin, which exhibits antibacterial activity.
Clindamycin, like most protein synthesis inhibitors, has a predominantly bacteriostatic effect, its effectiveness is associated with the length of time when the concentration of the active substance remains above the MIC (minimum inhibitory concentration) of the infectious agent.
Indications for use
Treatment of bacterial vaginosis (previous names: hemophilic vaginitis, gardnerella vaginitis, nonspecific vaginitis, corynebacterial vaginitis, or anaerobic vaginosis).
Contraindications
Hypersensitivity to the active substance, lincomycin or to any excipients listed in the "Composition" section.
Dalacin is also contraindicated in patients with a history of antibiotic-associated colitis.
Method of administration and dosage
Dosages.
The recommended dose is 1 suppository intravaginally before bedtime for 3 consecutive days.
Mode of application.
Dalacin is used intravaginally.
Introduction without an applicator.
Pull the suppository out of the foil.
Lie on your back and pull your knees to your chest.
Insert the suppository into the vagina with the help of the middle finger of the hand deeper, but so as not to cause discomfort.
If there is an applicator in the package, the patient can use it to facilitate the administration of the suppository.
Introduction with an applicator.
The plastic applicator, which is in the package with the preparation, is intended for the correct intravaginal administration of the suppository.
Pull the suppository out of the foil.
Place the flat end of the suppository into the opening of the applicator.
Lie on your back and pull your knees to your chest.
Holding the applicator by the ribbed end of the body, gently insert it into the vagina as deep as possible, but so as not to cause discomfort.
Slowly pressing the plunger, insert the suppository into the vagina.
Pull the applicator out of the vagina.
After each use, the applicator should be washed with warm water and soap and allowed to dry completely.
Overdose
No cases of overdose of the preparation Dalacin in the form of vaginal suppositories have been reported.
Clindamycin phosphate, contained in the preparation and administered vaginally, can be absorbed in an amount sufficient for the development of systemic effects.
In case of overdose, the use of general symptomatic and supportive treatment is indicated, if necessary.
In case of accidental ingestion, the occurrence of effects comparable to the effects of therapeutic concentrations of clindamycin in the case of oral administration is possible.
Special instructions
Use during pregnancy or lactation
Pregnancy.
Reproductive toxicity has been demonstrated in animal studies.
Application during the first trimester of pregnancy is not recommended, as there are no well-controlled studies on the use of the preparation in pregnant women during this period.
According to clinical studies of the use of the preparation Dalacin in a dosage form for vaginal use in women during the II trimester of pregnancy and the systemic use of clindamycin phosphate during the II and III trimesters did not lead to the development of congenital anomalies.
Dalacin can be used in the II and III trimester of pregnancy only if absolutely necessary. During pregnancy, it is recommended to administer vaginal suppositories without using an applicator.
Breastfeeding.
It is not known whether Dalacin penetrates into breast milk after vaginal administration. However, the appearance of clindamycin in breast milk has been reported after oral or parenteral administration. Consideration should be given to stopping breastfeeding or interrupting / refraining from therapy with Dalacin, given the benefits of breastfeeding for the baby and the benefits of treatment for the mother.
Fertility
In animal studies, there was no effect on fertility.
Children
The safety and efficacy of using the preparation Dalacin in the form of vaginal suppositories in children has not been established.

The effect of Dalacin on the ability to drive vehicles or work with other mechanisms is absent or insignificant.
