Pharmacological properties
The action of Fanigan fast gel is due to the components that make up its composition.
pharmacodynamics. Diclofenac is an NSAID with a pronounced anti-inflammatory, analgesic, and antipyretic effect. The main mechanism of action is the suppression of the biosynthesis of prostaglandins.
In case of inflammation caused by injuries or rheumatic diseases, the use of diclofenac leads to a decrease in the severity of pain, tissue edema and a reduction in the period of renewal of the functions of damaged joints, ligaments, tendons and muscles.
Menthol irritates the cold receptors. This is accompanied by a narrowing and a decrease in capillary permeability at the site of its application, a feeling of coolness. Menthol produces a local distracting and mild pain relieving effect.
Methyl salicylate is a derivative of salicylic acid and causes local irritation. By irritating the skin receptors, methyl salicylate leads to the creation and release into the blood of a large amount of biologically active substances that regulate pain sensitivity. Substance P is released from neurons. A decrease in the accumulation of substance P in the nerve endings leads to a decrease in the severity of pain. In addition, methyl salicylate, which belongs to the group of NSAIDs, inhibits the synthesis of prostaglandins due to inhibition of COX, which reduces edema and infiltration of inflamed tissues.
Flaxseed oil, the main component of which is a-linolenic acid (ethyl ester of unsaturated fatty acid), has an anti-inflammatory effect, antioxidant properties, and improves blood circulation at the site of application.
Pharmacokinetics. The action of Fanigan Fast gel begins within a few minutes after application to the skin. After 20-30 minutes, the effect of the gel reaches its maximum. When applied topically, less than 5% of the dose is absorbed, thus the systemic effect of the preparation is minimal.
Indications
Local treatment of myositis, fibrositis, sciatica, stretching of muscles and tendons, traumatic injuries of the musculoskeletal system, pain in muscles and joints during severe physical exertion, inflammatory and degenerative diseases of the joints (ostearthritis of the peripheral joints and spine), rheumatic lesions of the musculoskeletal system (bursitis , arthritis, tendosynovitis, tendonitis).
Application
For adults and children over the age of 14, apply 2–4 g of gel (4–8 cm strip) in a thin layer onto the skin and rub it in easily 2-3 times a day. the average daily dose is 10 g of gel, which corresponds to 100 mg of sodium diclofenac. the preparation should be applied to intact skin, avoid contact with eyes and mucous membranes. do not apply the gel to open wounds.
Hands should be washed after application of the preparation, unless the area has been treated.
The duration of treatment is determined by the doctor depending on the nature and course of the disease, the effectiveness of treatment.
The gel should not be used for more than 14 days in case of rheumatism and soft tissue damage, or more than 21 days in case of pain in arthritis, unless otherwise recommended by a doctor.
Elderly patients do not require dose adjustment.
Contraindications
Hypersensitivity to diclofenac or other components of the preparation. a history of BA attacks, urticaria or acute rhinitis due to the intake of acetylsalicylic acid and other NSAIDs. damage to the epidermis, open wounds in the area of application. the last trimester of pregnancy.
Side effects
From the immune system: hypersensitivity reactions (including urticaria), angioedema.
From the respiratory system: BA, bronchospasm, shortness of breath.
Skin and connective tissue disorders: irritation, rash (including pustular), eczema, erythema, dermatitis (including contact, bullous), itching, photosensitivity reactions, burning or tingling of the skin, dry skin.
Special instructions
Use with caution in conjunction with oral NSAIDs.
The likelihood of developing systemic side effects with topical application of diclofenac is insignificant compared with oral administration, but it is not excluded when the preparation is used on large areas of the skin for a long time.
Fanigan Fast gel should not be used with other products containing diclofenac. The preparation should be used with caution in case of peptic ulcer of the stomach and duodenum in the acute stage and its presence in history, impaired liver / kidney function, hematopoiesis disorder, polyposis of the nasal mucosa.
Fanigan Fast gel is not recommended to be applied to damaged skin, preventing contact with inflamed, wounded or infected skin. Avoid contact of the preparation with the eyes and mucous membranes. The preparation should not be swallowed. Fanigan Fast gel contains propylene glycol, which may cause mild localized skin irritation in some patients.
If any skin rash develops, treatment should be discontinued. Fanigan Fast Gel should not be used under an airtight occlusive dressing. In the event of a sprain, the affected area can be tied with a bandage.
Use during pregnancy and lactation. Clinical experience with pregnant women is limited, therefore, the use of this preparation is not recommended during pregnancy and lactation. Fanigan Fast gel is contraindicated in the third trimester of pregnancy due to the possibility of developing weakness in labor and / or premature closure of the ductus arteriosus.
If there is a good reason for using Fanigan Fast gel during lactation, when the expected benefit of the preparation, according to the doctor, outweighs the potential risk, the preparation should not be applied to the mammary glands or large areas of the skin and should not be used for a long time (more than 1 weeks).
Children. Contraindicated in children under 14 years of age. When using the preparation in children over the age of 14 for more than 7 days or if the symptoms of the disease intensify, you should seek the advice of a doctor.

Interactions
Since systemic absorption due to topical application of the preparation is very low, the likelihood of interactions is very low. however, it is necessary to consider possible interactions that are known for oral forms of diclofenac.
Overdose
Overdose is unlikely due to the low absorption of diclofenac into the systemic circulation when applied topically. in case of accidental ingestion, it should be borne in mind that 1 tube of the preparation, 100 g each, contains the equivalent of 1 g of sodium diclofenac; in this case, the development of systemic adverse reactions is possible.
If the preparation is accidentally swallowed, you should immediately empty the stomach and take the adsorbent. Symptomatic treatment is shown with the use of therapeutic measures used to treat NSAID poisoning.
