Description
Pharmacological properties
Pharmacodynamics. Xylometazoline is a sympathomimetic agent acting on α-adrenergic receptors.
Xylometazoline, when administered nasally, causes a narrowing of the blood vessels of the nasal mucosa and adjacent nasopharyngeal areas, thus eliminating edema and hyperemia of the nasal and nasopharyngeal mucosa, and also reduces the associated increased mucus production and facilitates the removal of blocked nasal secretions, which leads to clearing of the nasal passages and relief of nasal breathing.
The action of the preparation begins 2 minutes after application and lasts up to 12 hours.
The preparation is well tolerated, including by patients with sensitive mucous membranes, and does not reduce mucociliary function. Laboratory results indicate that xylometazoline reduces the infectious activity of human rhinovirus, which is associated with the common cold.
Thanks to the moisturizing components (sorbitol, hypromellose), the preparation Farmazolin with mint and eucalyptus, a nasal spray, helps with dryness and irritation of the mucous membrane of the nasal passages. The preparation has a balanced pH value within the range characteristic of the nasal cavity.
Pharmacokinetics. When applied topically, the preparation is almost not absorbed, the concentration of xylometazoline in the blood plasma is so low that it is almost undetectable (the concentration in the blood plasma is close to the detection limit).
Xylometazoline is not mutagenic. Also, in animal studies, no teratogenic effects of xylometazoline have been identified.
Indications
Symptomatic treatment of nasal congestion with colds, hay fever, other allergic rhinitis, sinusitis; to facilitate the outflow of secretions in diseases of the paranasal sinuses; auxiliary therapy in cases of otitis media (to eliminate swelling of the mucous membrane); to facilitate rhinoscopy.
Application
Adults and children over the age of 12: 1 injection into each nasal passage up to 3 times a day.
Apply no more than 3 times in each nasal passage per day. The duration of treatment depends on the course of the disease and should not exceed 10 consecutive days.
The dosed spray ensures dosing accuracy and proper distribution of the solution over the surface of the nasal mucosa. With each injection, 0.14 g of solution is injected, which corresponds to 0.14 mg of xylometazoline.
Before use, it is necessary to bring the dispensing device into readiness by performing several injection movements until the spray begins to be released into the air. On further use, the dispenser will be ready for immediate use.
It is necessary to apply the spray as follows:
thoroughly cleanse the nose before using the preparation ;
hold the bottle vertically, supporting the bottom with your thumb and placing the tip between two fingers;
tilt the bottle slightly and insert the tip into the nostril;
inject and at the same time take a light breath through the nose;
after use, before you close the tip with the cap, you should clean and dry the tip;
to prevent infection, only one person should use each bottle of the preparation.
The last application is recommended immediately before bedtime.
Contraindications
Hypersensitivity to xylometazoline or to any other component of the preparation, acute coronary diseases, coronary asthma, hyperthyroidism, angle-closure glaucoma, transphenoidal hypophysectomy and surgery with exposure of the meninges in history, dry rhinitis (rhinitis sicca) or atrophic rhinitis. concomitant treatment with MAO inhibitors and within 2 weeks after discontinuation of their use.
Side effects
From the immune system: rarely (1/10 000) - hypersensitivity reactions, including angioedema, rash, itching;
from the nervous system: often (from 1/100 to 1/10) - headache;
on the part of the organ of vision: rarely (1/10 000) - temporary visual impairment;
on the part of the cardiovascular system: rarely (1/10 000) - irregular or rapid heartbeat;
on the part of the respiratory system, chest and mediastinum: often (from 1/100 to 1/10) - dryness or discomfort on the part of the nasal mucosa;
from the digestive system: often (from 1/100 to 1/10) - nausea;
general disorders and reactions at the injection site: often (from 1/100 to 1/10) - a burning sensation at the site of application.
Special instructions
The preparation should not be used for more than 10 consecutive days. too long or excessive use can lead to recurrence of nasal congestion and / or atrophy of the nasal mucosa.
The preparation, like other sympathomimetics, should be prescribed with caution to patients who have strong reactions to adrenergic preparations, which are manifested by insomnia, dizziness, tremors, cardiac arrhythmias or increased blood pressure.
Do not exceed the recommended dose of the preparation, especially when treating children and the elderly.
Care should be taken to prescribe the preparation to patients with cardiovascular diseases, hypertension, diabetes mellitus, thyroid hyperfunction, pheochromocytoma, prostatic hypertrophy, and also not to use with concomitant treatment with MAO inhibitors and within 2 weeks after stopping their use.
The preparation contains benzalkonium chloride, which can cause bronchospasm.
Application during pregnancy or lactation. The preparation should not be used during pregnancy due to its potential vasoconstrictor effect.
There is no evidence of any unwanted influence on the infant. It is not known whether xylometazoline is excreted in breast milk, therefore, caution is needed, during breastfeeding, the preparation should be used only as directed by a doctor.
Fertility There are no adequate data on the effect of the preparation Farmazolin with mint and eucalyptus on fertility. Since systemic exposure to xylometazoline hydrochloride is very low, the likelihood of an effect on fertility is extremely low.
Children. The preparation should not be used in children under 12 years of age.
The ability to influence the reaction rate when driving or working with other mechanisms. Usually, the preparation does not have or has little effect on the ability to drive vehicles or work with other mechanisms.
Interactions
MAO inhibitors: Xylometazoline can potentiate the action of MAO inhibitors and induce a hypertensive crisis. do not use xylometazoline in patients who have taken or have taken MAO inhibitors within the past 2 weeks.
Tri- and tetracyclic antidepressants: with the simultaneous use of tri- or tetracyclic antidepressants and sympathomimetic preparations, it is possible to increase the sympathomimetic effect of xylometazoline, therefore, the simultaneous use of these preparations is not recommended.
When used with β-adrenergic receptor blockers, it can cause bronchial spasm or a decrease in blood pressure.
Overdose
Excessive local use of xylometazoline hydrochloride or its accidental ingestion can lead to severe dizziness, sweating, a significant decrease in body temperature, headache, bradycardia, arterial hypertension, respiratory depression, coma and seizures. an increased hell can change to a lower one. young children are more susceptible to overdose than adults.
All patients with suspected overdose should be prescribed appropriate supportive measures, as well as, if necessary, urgent symptomatic treatment under medical supervision. Medical care should include monitoring the patient for several hours. In case of severe overdose, accompanied by cardiac arrest, resuscitation should be continued for at least 1 hour.
