Description

Indications

Essential hypertension in patients who are recommended combination therapy (ramipril and hydrochlorothiazide).

Contraindications.

Hypersensitivity to the active substance ramipril or other ACE inhibitors (angiotensin-converting enzyme), hydrochlorothiazide, other thiazide diuretics, sulfonamides, or any of the excipients that make up the preparation (see Section "Composition").

A history of angioedema (hereditary, idiopathic, or previously transferred to the background of the use of ACE inhibitors or angiotensin II receptor antagonists).

Arterial hypotension or hemodynamically unstable conditions.

Simultaneous use of ACE inhibitors and extracorporeal therapies that lead to blood contact with negatively charged surfaces, since such use can lead to severe anaphylactic reactions. Such extracorporeal therapies include dialysis or hemofiltration using certain membranes with high hydraulic permeability (for example, polyacrylonitrile) and low-density lipoprotein apheresis using dextran sulfate (see Section "Interaction with other preparations and other types of interactions").

Significant bilateral renal artery stenosis or unilateral renal artery stenosis in the presence of a single kidney.

Severe renal impairment (creatinine clearance <30 ml/min) in patients not undergoing hemodialysis.

Clinically significant electrolyte imbalance, the course of which may worsen during treatment (see section "Peculiarities of use").

Treatment-resistant hypokalemia or hypercalcemia.

Refractory hyponatremia.

Symptomatic hyperuricemia (gout).

Anuria.

Severe liver dysfunction, hepatic encephalopathy.

Simultaneous use with preparations containing aliskiren in patients with diabetes, or in patients with moderate or severe renal impairment (creatinine clearance

<60 ml/min).

Simultaneous use with preparations of angiotensin II receptor antagonists in patients with diabetic nephropathy.

Compound

active ingredients: ramipril, hydrochlorothiazide;

1 tablet contains 5 mg of ramipril and 25 mg of hydrochlorothiazide;

excipients: lactose, hypromellose, crospovidone, microcrystalline cellulose, sodium stearyl fumarate.

Method of application and dose.

For oral use.

The preparation is recommended to be taken once a day at the same time, preferably in the morning.

The preparation can be taken before, during and after meals, since food intake does not affect the bioavailability of the preparation (see Section "Pharmacokinetics"). Tablets should be swallowed whole with water. They must not be chewed or crushed.

Adults. Adjust the dose individually, depending on the characteristics of the patient (see section "Peculiarities of use") and blood pressure levels. The use of a fixed combination of ramipril and hydrochlorothiazide is generally recommended only after dose titration of each of its individual components.

Start treatment with the lowest possible dose. If necessary, the dose can be gradually increased until the target blood pressure is reached. The maximum daily dose is 10 mg of ramipril and 25 mg of hydrochlorothiazide per day, which corresponds to 2 tablets of Hartil®-H 5 mg / 12.5 mg.

Children.

The preparation is not recommended for use in children, since there is insufficient data on its efficacy and safety for such patients.
