    Product description 

    Lerkamen 20 tablets are indicated for mild to moderate essential hypertension.

    Compound

    Active ingredient: 1 film-coated tablet contains lercanidipine hydrochloride 20 mg, which corresponds to lercanidipine 18.8 mg;
    Excipients: lactose, microcrystalline cellulose, sodium starch (type A), povidone K 30, magnesium stearate;
    Shell: Opadry 02F25077 (hypromellose, talc, titanium dioxide (E 171), macrogol 6000, iron oxide (E172)).

    Contraindications

    Hypersensitivity to the active substance or to any excipient of the preparation.
    Obstruction of vessels leaving the left ventricle.
    Untreated congestive heart failure.
    Unstable angina or recent (within 1 month) myocardial infarction.
    Severe liver dysfunction.
    Severe renal dysfunction (GFR <30 ml / min), including in patients on dialysis.
    Simultaneous use with: strong CYP3A4 inhibitors, cyclosporine, grapefruit or grapefruit juice.

    Mode of application

    Safety precautions to be observed when taking or handling the medicinal product:
    The preparation should be taken mainly in the morning, at least 15 minutes before breakfast.
    This medicine should not be taken with grapefruit juice.
    The recommended dose is 10 mg orally once a day at least 15 minutes before meals, the dose may be increased to 20 mg depending on the individual sensitivity of the patient. Dose titration should be gradual, since the maximum antihypertensive effect may occur 2 weeks after the start of treatment.

    Overdose

    As in the case of other dihydropyridines, an overdose of lercanidipine leads to excessive peripheral vasodilation with severe arterial hypotension and reflex tachycardia. However, when using the preparation in very high doses, a loss of peripheral selectivity is possible, which causes bradycardia and a negative inotropic effect. The most common adverse reactions caused by overdose were arterial hypotension, dizziness, headache and palpitations.
    Treatment. Patients with clinically significant arterial hypotension require active cardiovascular support, including frequent monitoring of cardiac and respiratory function, a horizontal position with elevated lower limbs, control of circulating fluid volume and diuresis

    Side effects

    The most commonly reported adverse reactions in clinical trials and post-marketing surveillance are peripheral edema, headache, flushing, tachycardia, and palpitations.

    Interaction

    Contraindicated simultaneous use with: strong CYP3A4 inhibitors, cyclosporine, grapefruit or grapefruit juice.
    Alcohol should be avoided as it may enhance the effect of vasodilatory antihypertensive preparations.
