    Pharmacological properties

    Combined mucolytic agent containing ambroxol hydrochloride (4 - {[(2-amino-3, 5-diphenyl) methyl] amino} cyclohexanol hydrochloride) and carbocysteine ​​((2r) -2-amino-3 - [(carboxymethyl) sulfanyl] propane acid).
    Ambroxol is an active metabolite of bromhexine; in contrast to the original compound, it is more active. Non-toxic, has a wide therapeutic range. It is used for acute and chronic diseases of the respiratory tract, which are accompanied by the formation of viscous, difficult to separate sputum. It normalizes the pathologically altered secretion of glandular cells of the bronchial mucosa, promotes thinning of viscous secretion and facilitates its discharge by increasing mucociliary clearance and changing the ratio of serous and mucous components of sputum, restoring the drainage function of small bronchi and bronchioles. Stimulates Clarke cells and activates hydrolyzing enzymes, which also leads to a decrease in sputum viscosity. Stimulates the formation of endogenous surfactant, without causing excessive secretion, reduces spastic hyperreactivity of the bronchi. Cough and sputum volume are significantly reduced when using the preparation. It is well tolerated even with prolonged use.
    Carbocisteine ​​has a mucolytic and expectorant effect due to the activation of sialic transferase, an enzyme of the goblet cells of the bronchial mucosa. Normalizes the quantitative ratio of acidic and neutral sialomucins, which helps to reduce the viscosity of bronchial secretions. Facilitates sputum separation by increasing mucociliary clearance, has antioxidant and pneumoprotective properties, which are due to the ability of sulfhydryl groups to bind free radicals. It also promotes the regeneration of the mucous membrane and the normalization of its morphological structure, activates the function of the ciliated epithelium, restores the secretion of immunoglobulin and the number of sulfhydryl groups of mucus components.
    After taking ambroxol, it is almost completely absorbed in the digestive tract, it penetrates well into the lung tissue. Absolute oral bioavailability is 70-80%. The maximum concentration in blood plasma is reached approximately 2 hours after ingestion. The half-life is 9-10 hours. It is excreted mainly in the urine.
    The maximum concentration of carbocysteine ​​in the blood plasma and the mucous membrane of the respiratory tract is achieved in 1-3 hours; in the mucous membrane, it persists for 8 hours. It is excreted almost completely in the form of inactive metabolites (inorganic sulfates, diacetylcystine) in the urine. Only a small amount is excreted unchanged in the feces. It can penetrate the placental barrier and accumulate in the amniotic fluid.

    Indications

    Acute and chronic diseases of the respiratory tract, which are accompanied by the formation of hard-to-separate secretions: chronic obstructive bronchitis, pneumonia, BA, bronchiectasis, cystic fibrosis, shock lung syndrome in adults, for the prevention and treatment of complications after lung surgery, for tracheostomy care, before and after bronchoscopy, inflammatory diseases of the middle ear and paranasal sinuses.

    Application

    Contraindications

    Hypersensitivity to the components of the preparation, peptic ulcer of the stomach or duodenum, the first trimester of pregnancy, a history of convulsions, chronic glomerulonephritis in the acute phase.

    Special instructions

    The use of the preparation during pregnancy is possible only if the expected therapeutic effect for the mother outweighs the potential risk to the fetus. it is advisable to avoid prescribing the preparation during lactation (the components of the preparation can penetrate into breast milk). with caution prescribed for hepatic and renal failure.

    Interactions

    Increases the effectiveness of therapy with corticosteroids and antibacterial agents in inflammatory diseases of the upper and lower respiratory tract. it is not recommended to use it simultaneously with tetracycline antibiotics (with the exception of doxycycline); the interval between taking them should be at least 2 hours.
    The simultaneous use of ambroxol and carbocisteine ​​with antitussive preparations (for example, codeine) is not recommended due to the possible accumulation of secretions in the respiratory tract when the cough reflex is inhibited.

    Overdose
