    Pharmacological properties

    Pharmacodynamics. Phenylephrine hydrochloride is a synthetic adrenergic agonist. stimulating α-adrenergic receptors of blood vessels, it promotes a pronounced vasoconstrictor effect. the vasoconstrictor effect is manifested by a decrease in blood flow, a decrease in edema of the mucous membranes of the nose, paranasal sinuses and the Eustachian tube. thereby restoring nasal breathing, which was disturbed by flu, colds and allergic diseases.
    Local vasoconstriction of the mucous membranes of the nose and paranasal sinuses occurs 3-5 minutes after the introduction of the preparation into the nasal cavity. The decongestant effect lasts up to 4-6 hours. The glycerin contained in the preparation softens the irritated mucous membrane of the nasal passages and protects it from excessive drying out.
    Pharmacokinetics. A preparation for topical use. When absorbed in the gastrointestinal tract, MAO is metabolized in the liver and intestines.

    Indications

    Treatment of acute rhinitis, sinusitis, acute otitis media (as an adjuvant) in children aged 2 months to 6 years.

    Application

    To bury in each nasal passage for children aged 2 months to 1 year - 1 drop; from 1 to 2 years - 1-2 drops each; from 2 to 6 years - 2-3 drops. repeat not earlier than after 4 hours, but not more often than 4 times a day.
    Children aged 6 years and older are expected to use more concentrated phenylephrine solutions (0.25%), for example Nazol® Kids.
    The duration of the course of treatment usually does not exceed 3 days.

    Contraindications

    Increased individual sensitivity to the components of the preparation (allergic reactions), a significant increase in blood pressure, tachysystolic heart rhythm disturbances, acute cardiovascular diseases, decompensated heart failure, cardiac conduction disturbances, renal failure, severe thyrotoxicosis, acute pancreatitis, hepatitis, inflammation of the skin and mucous membranes the membranes of the anterior nasal cavity and the tendency to form nasal crusts (atrophic rhinitis), conditions after transsphenoidal hypophysectomy; simultaneous use with MAO inhibitors or use within 15 days after stopping treatment with MAO inhibitors.

    Side effects

    In isolated cases, such side effects are possible.
    At the site of application: irritation at the site of application, sneezing, burning, tingling in the nose.
    From the side of the cardiovascular system: flushing of the face, heart rhythm disturbances, bradycardia, increased blood pressure.
    From the nervous system: dizziness, fear, headache.
    From the immune system: allergic reactions, including cross-reactions in case of allergy to other sympathomimetics, which can manifest as rash, itching, urticaria, angioedema (hands, face or throat), a feeling of heaviness in the chest, shortness of breath.
    When using the preparation in doses exceeding the recommended ones, the manifestations indicated in the OVERDOSE section are possible.

    Special instructions

    Before using the preparation, the patient's head must be tilted back, then turn the bottle over, holding it over the nasal passage. in this position, the preparation is released in drops. it is not recommended to use the same bottle for several persons in order to avoid the spread of infection.
    The persistence of swelling of the nasal passages after 3 days of use may indicate the presence of curvature of the nasal septum, purulent sinusitis, adenoids, allergic rhinitis, persistent bacterial infection or other diseases requiring the consultation of a specialized specialist and specialized complex therapy.
    This preparation should be used with caution in patients with hypertension, cardiovascular diseases, thyroid diseases (except for those indicated in the CONTRAINDICATIONS section, angle-closure glaucoma, diabetes mellitus).
    The preparation should be used under adult supervision.
    If children under the age of 12 develop nervousness, dizziness or insomnia, discontinue use of the preparation and consult a doctor.
    Use during pregnancy and lactation. Do not use.
    Children. Should not be used in children under 2 months of age.
    The ability to influence the reaction rate when driving or working with other mechanisms. This preparation is not expected to affect the reaction rate when driving or using other mechanisms.

    Interactions

    It should not be used with other vasoconstrictor agents (for any route of administration of the latter), as well as with antihypertensive preparations (β-adrenergic receptor blockers).
    Atropine sulfate blocks phenylephrine-induced reflex bradycardia and increases the vasopressor response to phenylephrine. Simultaneous administration of phenylephrine with β-blockers can lead to hypertension and excessive bradycardia with possible heart block. It should be used with caution with thyroid hormones, preparations that affect cardiac conduction (cardiac glycosides, antiarrhythmic preparations). With simultaneous use with preparations that cause the excretion of potassium, for example, with some diuretics such as furosemide, hypokalemia may increase and arterial sensitivity to vasopressor preparations such as phenylephrine may decrease.
    The simultaneous administration of phenylephrine and other sympathomimetics can lead to additional stimulation of the central nervous system to an extremely high level, which is accompanied by nervousness, irritability, and insomnia. Seizures are also likely. In addition, the simultaneous use of other sympathomimetics together with phenylephrine can lead to an increase in the vasoconstrictor effect or cardiovascular effect of either of these two medications.
    Avoid concomitant use or start taking this preparation at least 15 days after interrupting therapy with MAO inhibitors, since pressor effects may increase (see CONTRAINDICATIONS).
    The simultaneous use of phenylephrine with linezolid is not recommended.

    Overdose

    Although with local application, side systemic reactions usually do not occur, however, in case of an overdose, an increase in the manifestations of side effects is possible, especially with prolonged use. cardiac arrhythmias, increased blood pressure, arterial hypotension, pain and discomfort in the region of the heart, palpitations, shortness of breath, noncardiogenic pulmonary edema, agitation, convulsions, headache, tremors, sleep disturbances, anxiety, anxiety, nervousness, irritability, inappropriate behavior , psychosis with hallucinations, insomnia, weakness, anorexia, nausea, vomiting, oliguria, urinary retention, painful or difficult urination, facial flushing, feeling of coldness in the extremities, paresthesia, skin pallor, piloerection, increased sweating, hyperglycemia, vascularity, hypokalemia and vessels of internal organs, a decrease in blood flow to vital organs, which can lead to a deterioration in the blood supply to the kidneys, metabolic acidosis, and an increase in the load on the heart due to an increase in the ovarian pressure. severe consequences of vasospasm can be more common in patients with hypovolemia and severe bradycardia. there is no specific antidote for phenylephrine poisoning. symptomatic treatment.
