    Pharmacological properties

    Pancreatin is a powder from porcine pancreas that contains enzymes such as lipase, amylase, trypsin and chymotrypsin to help digest proteins, fats and carbohydrates in the digestive tract. the key ones are the enzymatic activity of lipase, as well as the content of trypsin, while amylolytic activity is important only in the treatment of cystic fibrosis, since even with a significant decrease in the secretory activity of the pancreas, amylase exhibits sufficient activity.
    The acid-resistant membrane protects active enzymes from the action of gastric juice. Only under the action of the neutral or slightly alkaline medium of the small intestine does the membrane dissolve and the release of enzymes occurs.

    Indications

    Chronic pancreatitis, cystic fibrosis. conditions after simultaneous resection of the stomach and small intestine, with a functional acceleration of the passage of food through the intestines, with intestinal disorders, with the simultaneous use of indigestible vegetable, fatty and unusual food. panzinorm forte 20,000 is also used for intestinal distention and in preparation for X-ray or ultrasound diagnostic examinations.

    Application

    The dose of panzinorm forte 20,000 is determined individually, depending on the severity of pancreatic insufficiency. The usual starting dose is 1 to 2 tablets per meal. the tablet is washed down with a sufficient amount (glass) of water. depending on the type of food, as well as on the degree of indigestion, the dose can be significantly increased. increasing the dose should be carried out under the supervision of a physician and at the same time focus on reducing the severity of the manifestations of the disease, for example, steatorrhea or pain. the maximum daily dose for adults and children over the age of 15 is 9 tablets. the question of increasing the dose of the preparation and the duration of treatment is decided by the doctor individually.

    Contraindications

    In case of hypersensitivity to pork or other components of the preparation; with acute pancreatitis or an acute attack of chronic pancreatitis in the phase of its exacerbation.

    Side effects

    In patients with cystic fibrosis, especially when using the preparation in high doses, an increased excretion of uric acid in the urine may be noted, therefore, to prevent the formation of uric acid calculi in such patients, it is necessary to control the level of uric acid in the urine. in some cases, after taking pancreatin, immediate allergic reactions have been reported, for example, skin rash, sneezing, lacrimation, bronchospasm. when using pancreatin in high doses, strictures can form in the ileocecal and ascending part of the large intestine in patients with cystic fibrosis.

    special instructions

    Intestinal obstruction is a known complication in patients with cystic fibrosis, therefore, if there are symptoms that resemble this condition, one should be aware of the possibility of intestinal strictures.
    The preparation contains active enzymes that can damage the mucous membrane of the oral cavity, so the tablets should be swallowed whole, without chewing, with a sufficient amount of liquid.
    The preparation contains lactose. Therefore, it should not be used in patients with rare hereditary disorders, such as galactose intolerance, congenital lactase deficiency or glucose-galactose malabsorption syndrome.
    Application during pregnancy or lactation. The preparation is recommended to be used only as directed by a doctor in the case when the expected benefit to the mother outweighs the potential risk to the fetus or child.
    The ability to influence the reaction rate when driving or operating other mechanisms. There was no effect on the ability to drive vehicles and work with mechanisms.
    Children. It is not recommended for use in children under 15 years of age, since the tablet is film-coated and cannot be divided.

    Interactions

    Pancreatic enzymes inhibit the absorption of folic acid, which may require additional intake of folic acid.
    Pancreatic enzymes can reduce iron absorption, but the clinical significance of this interaction is unknown.
    The acid-resistant film shell of Panzinorm forte 20,000 tablets is destroyed in the duodenum. If the acidity of the contents of the duodenum is too high, the enzymes will be released out of time. A decrease in gastric acid secretion is achieved by the use of H2-receptor inhibitors or proton pump inhibitors, increases the efficiency of enzyme intake in some patients and makes it possible to reduce the dose of Panzinorm forte 20,000.

    Overdose

    There is no data on systemic intoxication in case of overdose. overdose can cause nausea, vomiting, diarrhea, hyperuricemia, hyperuricosuria and uricosuria, perianal pruritus, and very rarely, mainly only in patients with cystic fibrosis, fibrous colonopathy. in case of an overdose, taking the tablets should be discontinued, hydration of the body and symptomatic treatment are recommended.
