Pharmacological properties
Pharmacodynamic parameters. The combination of active pharmaceutical ingredients of the Polydex preparation determines its anti-inflammatory and antimicrobial effects against a wide range of gram-positive and gram-negative pathogens of the external ear of an infectious and inflammatory nature.
Synthetic GCS dexamethasone penetrates the cell membrane and binds with a high affinity to specific cytoplasmic receptors of glucocorticoids, which ultimately leads to a modification of transcription and, consequently, protein synthesis to achieve inhibition of leukocyte infiltration at the site of inflammation, interference with the function of inflammatory mediators, suppression of humoral immune reactions and reduction of edema. The anti-inflammatory effect of dexamethasone involves the phospholipase A2 inhibiting proteins lipocortins, which control the biosynthesis of powerful inflammatory mediators such as prostaglandins and leukotrienes.
Neomycin is an aminoglycoside antibiotic isolated from cultures of Streptomyces fradiae. Its mechanism of action includes inhibition of the synthesis of proteins vital for the growth of microbial cells by binding to the 30S subunit of the bacterial ribosome. Neomycin can also inhibit bacterial DNA polymerase.

Neomycin-resistant enterococci, streptococci, methicillin-resistant staphylococci, chlamydia, mycoplasma, rickettsia, P. stuartii, Pseudomonas aeruginosa, A. denitrificans, burkholderia, N. asteroides, flavobacteria, resistant microorganisms.
Polymyxin B, a lipopeptide antibiotic isolated from Bacillus polymyxa, is bactericidal against gram-negative bacteria: the α- and γ-diaminobutyric acids of the positively charged polymyxin B form electrostatic interactions with the phosphate groups of negatively charged lipid A on the outer membrane of the gram-negative bacteria. Calcium and magnesium ions are displaced from the phosphates of membrane lipids, destabilizing the lipopolysaccharide, which increases the permeability of the membrane and, as a result, leads to cell destruction. In addition, polymyxin B inhibits type II NADH-quinone oxidoreductase required for cellular respiration on the inner bacterial membrane, and can also bind and neutralize lipopolysaccharide released during bacterial lysis, preventing reactions to endotoxin.
Acinetobacteria, aeromonads, Alcaligenes spp., Microorganisms of the genera Citrobacter, Enterobacter, Escherichia coli, Klebsiella, Moraxella, Pseudomonas aeruginosa, Salmonella, Shigella, S. maltophilia are sensitive to polymyxin B.
Show resistance to polymyxin B of Brucella, Burkholderia (B. cepacia, B. pseudomallei), Mycobacterium, B. catarrhalis, Campylobacter, Legionella, Morganella, Proteus, Neisseria, Serracias, Providence, C. meningosepticum, Cholerae Vibrio and Shavarovidia rod-shaped spore-forming bacteria.
Pharmacokinetic parameters. In the case of an intact tympanic membrane, there is no absorption into the systemic circulation.
Indications
Topical therapy for external otitis media of bacterial etiology in the absence of damage to the tympanic membrane (including eczema of the external auditory canal of an infectious nature).
Application
Locally (instillation into the external auditory canal) 2 times a day - in the morning and in the evening.
Adults 1-5 drops, children 1-2 drops (depending on age).
The duration of the course of therapy (on average 1 week) is individual and is determined by the severity of the pathology and its nature.
Before use, warm the bottle in your hands to body temperature. Tilt your head (and keep in this position for another 5 minutes after instillation); drip the preparationinto the external auditory canal, not under pressure, then slightly pull the earlobe (repeat several times).
At the end of the course of therapy, dispose of the vial, do not reuse.
Contraindications
Hypersensitivity to active pharmaceutical ingredients or other components of the preparation;
the presence (or assumption of the presence) of a violation of the integrity of the tympanic membrane;
infectious process of the external auditory canal of viral etiology (including chickenpox and herpes simplex).
Side effects
Allergic reactions and sensitization to antibiotics as part of the Polydex preparation; contact dermatitis, local allergic reaction and skin irritation; with perforation of the tympanic membrane - vestibular or cochlear ototoxic effects; the development of microorganisms and mycoses insensitive to antibiotics in the composition of the Polydex preparation.
Special instructions
Before starting therapy, an examination of the tympanic membrane is required.
The solution is not administered orally or by injection.
During therapy, it is necessary to adhere to the approved instructions for the proper use of antibacterial agents.
The presence of GCS in the preparation can affect the symptoms of an allergic reaction.
If an allergic reaction develops, the preparation should be discontinued immediately.
Dexamethasone in the composition of the preparation can cause a positive reaction to a doping test.
To minimize the likelihood of microbial contamination, it is necessary to prevent contact of the skin of the hand or ear with the edge of the dropper.
If the manifestations of the disease persist after 10 days of therapy, a second medical consultation is required.
With prolonged therapy or the use of doses of the preparation in excess of the recommended, systemic effects may develop.
Application during pregnancy and lactation is possible after a careful assessment of the prevalence of the benefits to the mother over the possible risk to the fetus / child.
Children have no age restrictions.
Does not affect the ability to drive vehicles or operate other mechanisms.
Interactions
Combined use with any other topical preparations is undesirable.
