Posterisan ointment is a specialized immunomodulatory preparation designed to address the distressing symptoms associated with hemorrhoidal disease and perianal inflammation. Unlike many conventional topical treatments that rely primarily on corticosteroids or local anesthetics for temporary symptom masking, Posterisan utilizes a standardized suspension of bacterial culture (Escherichia coli) to stimulate the body’s natural local defense mechanisms. This biological approach not only alleviates itching and burning but also promotes the regeneration of damaged tissue and strengthens the resistance of the anal mucosa to pathogenic bacteria. It is an ideal solution for patients seeking a treatment that supports the body's own healing processes for long-term relief from chronic anal discomfort, fissures, and eczema.

    Composition and Active Substance

Each 1 gram of Posterisan ointment contains:

    • Active Substance: 166.7 mg of a standardized suspension of bacterial culture (CSBC), which includes at least 500 million inactivated Escherichia coli bacteria, their cell wall components, and metabolic products.

    • Preservative: 3.3 mg of Liquefied Phenol.

    • Excipients: Lanolin (wool fat) and white soft paraffin (petrolatum).

    Pharmacological Action

The efficacy of Posterisan is based on its local immunostimulating effect. When applied to the affected area, the inactivated E. coli components act as antigens:

    1. Leukocyte Activation: It stimulates the accumulation of T-lymphocytes and macrophages at the site of application, enhancing the local cellular immune response.

    2. Antibody Production: It promotes the synthesis of specific antibodies that protect the mucosa against secondary infections.

    3. Tissue Repair: By stimulating the immune system, it accelerates the natural healing of wounds and fissures, reducing inflammation and the associated swelling.

    4. Vascular Support: It helps stabilize the vascular walls in the hemorrhoidal plexus, reducing the tendency for bleeding and congestion.

    Indications for Use

    • Hemorrhoidal Disease: Particularly effective in the early stages and for recurring episodes.

    • Anal Pruritus: Relief of persistent itching in the perianal area.

    • Anal Fissures: Promotion of healing in small tears of the anal lining.

    • Perianal Eczema: Treatment of skin inflammation and irritation surrounding the anus.

    • Post-Surgical Recovery: Used to support tissue regeneration following proctological operations.

    Contraindications

    • Hypersensitivity: Known allergy to Escherichia coli bacterial components, phenol, or lanolin.

    • Specific Infections: If the affected area has a primary infection caused by fungi, viruses (e.g., herpes), or specific bacteria (e.g., tuberculosis or syphilis), Posterisan should only be used after the primary infection is controlled.

    Dosage and Administration

Posterisan ointment is typically applied twice daily (morning and evening), preferably after a bowel movement and thorough hygiene of the anal area.

    • External Use: Apply a thin layer of ointment to the affected skin and rub in gently.

    • Internal Use: For internal hemorrhoids, use the provided applicator. Screw the applicator onto the tube, insert it into the anus, and gently squeeze the tube to dispense the ointment.

    Special Administration Tips

For the best results, treatment should continue for several days after the symptoms have completely subsided to prevent recurrence. The duration of use is generally not limited, but if no improvement is seen within 5–7 days, a consultation with a proctologist is recommended.

    Side Effects and Adverse Reactions

Posterisan is exceptionally well-tolerated. However, some users may experience:

    • Local Reactions: A temporary stinging or burning sensation immediately after application, often due to the phenol preservative.

    • Allergic Reactions: In rare cases, hypersensitivity to lanolin or other components may cause redness, itching, or skin rashes. If severe irritation occurs, discontinue use.

    Special Precautions & Warnings

    • Pregnancy and Breastfeeding: There are no known risks to the fetus or infant, as the active ingredients act locally and are not absorbed systemically in significant amounts. However, consult a physician before use during pregnancy.

    • Latex Compatibility: The ointment base contains fats (paraffin and lanolin). If used with latex condoms, it may reduce the strength and reliability of the condom.

    • Hygiene: Thoroughly clean the applicator after each use to prevent bacterial contamination of the tube.

    Storage Conditions and Shelf Life

    • Store the tube at room temperature, ideally between 15°C and 25°C.

    • Do not freeze the ointment.

    • Shelf Life: 3 years. After the first opening of the tube, the product remains stable for the remainder of its shelf life if kept tightly closed.

    • Keep out of reach of children.
