Description
Pharmacological properties
Pharmacodynamics. Benzocaine reversibly stabilizes the neuron membrane, which reduces its permeability to sodium ions. depolarization of the neuron membrane is suppressed, and as a result, the emergence and conduction of nerve impulses is blocked. thus, benzocaine has a pronounced local analgesic effect.
Pharmacokinetics. A preparation for topical use. Benzocaine, which is part of it, is excreted in mucus or, in extremely small quantities, by the kidneys.
Indications
Relief® Advance, suppositories: external and internal hemorrhoids, cracks, erosion and microtrauma in the anus, perianal itching with severe pain syndrome. As an anesthetic after proctological surgery.
Application
The preparation should be used after hygiene procedures. Introduce 1 suppository into the anus in the morning, after each bowel movement and at bedtime. The daily dose is no more than 4 times a day. Regular use of the preparation provides a stable therapeutic effect, relieves the symptoms of hemorrhoids. The duration of treatment is usually 1 week. If necessary, it can be extended up to 3 weeks or more (after consulting a doctor).
Children. The efficacy and safety of Relief® Advance in children under the age of 12 have not been studied. No data available. For children under 12 years of age, Relief® Advance suppositories are prescribed by a doctor only if absolutely necessary.
Contraindications
Individual hypersensitivity to any component of the preparation and other amide local anesthetics, thromboembolic disease, granulocytopenia.
Side effects
From the side of the blood and lymphatic system: when using the preparation, methemoglobinemia is possible (cyanosis of the skin, lips and nail beds, headache, dizziness, shortness of breath, weakness, tachycardia).
General disorders and changes at the site of application: allergic reactions, including flushing, skin rash, itching, changes at the site of administration (irritation, swelling, pain).
On the part of the skin and subcutaneous tissue: allergic dermatitis, contact dermatitis.
Excipients methyl parahydroxybenzoate and propyl parahydroxybenzoate can cause allergic reactions (possibly delayed).
Special instructions
In case of profuse bloody discharge from the anus or if there are symptoms of the disease within 7 days of treatment, or if the condition worsens, it is necessary to additionally consult with a proctologist.
You should consult a doctor before starting to use Relief® Advance suppositories, and Relief® Advance ointment should be used with caution as prescribed by a doctor in the following cases: severe hypertension, severe cardiac arrhythmias, clinically expressed thyrotoxicosis, urinary disorders.
It is necessary to use the minimum required amount of Relief® Advance suppositories. Unless otherwise instructed by the doctor, do not exceed the recommended daily dose of Relief® Advance ointment to minimize systemic adverse reactions. Relief® Advance ointment is not recommended to be applied to damaged skin, it can promote systemic absorption. In case of bleeding from the anus, it is necessary to see a doctor as soon as possible.
There are reports that the administration of preparations containing benzocaine can cause methemoglobinemia. Symptoms such as cyanosis of the skin, lips and nail beds, headache, dizziness, shortness of breath (shortness of breath), weakness, tachycardia observed during treatment may indicate methemoglobinemia, which is potentially life threatening and requires urgent medical attention.
Propylene glycol can irritate the skin. Methylparaben and propylparaben can cause allergic reactions (possibly delayed).
Use during pregnancy and lactation. During pregnancy, the preparation should be used only if absolutely necessary after a doctor's weighted assessment of the benefit to the mother / risk to the fetus or child.
During breastfeeding, a decision should be made to stop breastfeeding or to stop using the preparation, taking into account the importance of taking the preparation to the mother.
The ability to influence the reaction rate when driving or working with other mechanisms. Relief® Advance, suppositories: unknown. Relief® Advance, ointment: there was no effect on the ability to drive vehicles or work with other mechanisms.
Interactions
It is not recommended to use the preparation without strict medical indications and medical supervision against the background of regular intake of MAO inhibitors, antidepressants and antihypertensive preparations due to the theoretical possibility of a decrease in the severity of the effect of MAO inhibitors or an increase in the effect of antihypertensive preparations due to interaction with benzocaine in cases of significant excess of the recommended doses and duration of administration.
Overdose
Overdose symptoms may be associated with the pharmacological effects of benzocaine. its systemic absorption in case of overdose can manifest itself as drowsiness, anxiety, heart rhythm disturbances, agitation. in severe cases, seizures, coma, decreased respiratory rate, or respiratory distress may occur. there are reports that the administration of products containing benzocaine can cause methemoglobinemia. Symptoms such as cyanosis of the skin, lips and nail beds, headache, dizziness, shortness of breath (difficulty breathing), weakness, tachycardia that may occur during treatment may indicate methemoglobinemia, which is potentially life threatening and requires urgent medical attention.
Emergency care in case of overdose and the development of methemoglobinemia consists in intravenous administration of methylene blue. Other symptoms of an overdose require discontinuation of the preparation and symptomatic treatment.
Storage conditions
At a temperature not exceeding 25 ° C. the opening of the tube and the beginning of using the ointment do not affect the shelf life, provided that it is tightly closed with a cap each time after use. do not use suppositories if the protective plastic sheath is missing or damaged.
