Product description
Oral solution "Rispetril" is used for the following indications:
treatment of schizophrenia and other mental disorders, including maintenance therapy in patients who have responded to therapy, in order to prevent relapse of the disease;
short-term treatment of severe aggression or severe mental symptoms in patients with dementia when there is a threat of harm to themselves or others;
treatment of manic episodes in bipolar disorders (auxiliary therapy in combination with mood stabilizers as initial treatment or as monotherapy for up to 12 weeks);
symptomatic treatment of social behavior disorders that cause oppositional disorders or other social behavior disorders in children, adolescents and adults with mental development below average or mental retardation, who have manifestations of destructive behavior (impulsivity, auto-aggression);
symptomatic treatment of autistic disorders in children aged 5 years and over whose symptoms range from hyperactivity to irritability (including aggression, self-harm, anxiety, and pathological cycling).
Compound
The active substance is risperidone (1 ml of solution contains 1 mg of risperidone).
Excipients: purified water, benzoic acid (E 210), tartaric acid, sodium hydroxide, sorbitol solution (E 420).
Contraindications
Severe sensitivity to the active substance or to any of the excipients of the preparation.
Mode of application
"Rispetril" can be used once or twice a day. Doses greater than 8 mg should be divided into two doses (morning and evening).
Schizophrenia
Adults (under the age of 65). "Rispetril" can be administered once or twice a day. You should start taking 2 mg of the preparation "Rispetril" per day, on the second day the dose can be increased to 4 mg. Thereafter, the dose can be maintained unchanged or, if necessary, adjusted individually. For most patients, the recommended dose is 4-6 mg per day. Some patients may need to gradually increase the dose or reduce the initial dose. The maximum daily dose is 10 mg.
Doses that exceed 10 mg per day have not been found to be highly effective compared to lower doses, but they may cause extrapyramidal symptoms. Since the safety of doses exceeding 16 mg per day has not been studied, doses above this level should not be used.
Short-term therapy for severe aggression or severe psychiatric symptoms in patients with dementia
The recommended starting dose is 0.25 mg twice daily. If necessary, the dose can be increased by increasing the dose of 0.25 mg twice a day no more than every other day. For most patients, the optimal dose is 0.5 mg twice daily. However, for some patients, the effective dose can be increased to 1 mg twice a day. Once the optimal dose has been reached, a once-daily daily dose may be considered.
Cancellation of treatment with the preparation "Rispetril" should take place no later than three months after the start of therapy, therapy can be resumed only if behavioral disorders reappear.
Manic episodes in bipolar disorder (adults and children over 10 years of age)
The recommended initial dose of Rispetril is 2 mg once a day, in the evening. The dose can be individually increased by adding 1 mg/day no more than every 24 hours. The recommended dose range is 2 to 6 mg per day.
As with other types of symptomatic treatment, with long-term use of the preparation "Rispetril" it is necessary to periodically review the doses and adjust them throughout therapy. There are no data on the effectiveness of the preparation "Rispetril" in the treatment of acute bipolar mania lasting more than 12 weeks. If "Rispetril" is used in combination with mood stabilizers, therapy can be stopped earlier, since the onset of the effect of treatment can be expected in the first weeks of therapy. Even after the first response to treatment, the possibility of recurrence of symptoms of depression should be taken into account due to the nature of the course of the disease and the adverse reactions of the preparations that were used for treatment, including Rispetril.
Symptomatic treatment of social behavior disorders or aggressive behavior
Patients weighing > 50 kg. The recommended starting dose is 0.5 mg once daily. If necessary, the dose should be adjusted by adding 0.5 mg once a day no more than every other day. The optimal dose for most patients is 1 mg once daily. However, for some patients, no more than 0.5 mg once a day is sufficient to achieve a positive effect, while others may require 1.5 mg once a day.
Patients (children from 5 years of age and adults) weighing < 50 kg. The recommended starting dose is 0.25 mg once daily. If necessary, the dose can be adjusted by adding 0.25 mg once a day no more than every other day. The optimal dose for most patients is 0.5 mg once daily. However, for some patients, no more than 0.25 mg once a day is sufficient to achieve a positive effect, while others may require 0.75 mg once a day.
As with other types of symptomatic treatment, long-term use of the preparation "Rispetril" must be periodically reviewed and adjusted throughout therapy.
Autism (children from 5 years old)
The dose should be selected individually, depending on the patient's condition and clinical response.
Patients weighing < 50 kg. The recommended starting dose is 0.25 mg once daily. From the 4th day, the dose can be increased by 0.25 mg. A dose of 0.5 mg should be maintained and clinical response assessed on day 14. Dose increases of 0.25 mg at two-week intervals should only be considered in patients with insufficient clinical response.
Patients weighing ≥ 50 kg. The recommended starting dose is 0.5 mg once daily. From the 4th day, the dose can be increased by 0.5 mg. Application features
pregnant
"Rispetril" is not recommended for use during pregnancy, except in cases of vital necessity. If it is necessary to stop treatment with Rispetril during pregnancy, this should not be done suddenly.
If necessary, breastfeeding should be discontinued.
Children
The preparation is used exclusively for the symptomatic treatment of social behavior disorders, defiant oppositional disorders or other social behavior disorders, as well as autistic behavioral disorders in children aged 5 years and over.
Drivers
Rispetril may have a minor or moderate effect on the ability to drive vehicles through potential effects on the nervous system and vision. Therefore, patients should refrain from driving vehicles and working with other mechanisms until their individual tolerability of the preparation is known.
Overdose
Symptoms
The signs and symptoms of overdose that have been observed are the known adverse reactions of the preparation, which manifest themselves in an enhanced form: drowsiness and sedation, tachycardia and arterial hypotension, as well as extrapyramidal symptoms. In case of overdose, QT interval prolongation and convulsions have been reported. Flutter-flicker has been reported associated with an overdose of Rispetril in combination with paroxetine.
Treatment
An airway must be established and maintained to ensure adequate ventilation and oxygenation. Consider gastric lavage (after intubation if the patient is unconscious) and administration of activated charcoal along with a laxative if less than an hour has passed after taking the preparation. Cardiac function monitoring is indicated, including continuous ECG recording to detect possible arrhythmias.
"Rispetril" does not have a specific antidote. Therefore, appropriate supportive measures should be applied. Arterial hypotension and circulatory collapse should be treated with such measures as intravenous infusions and / or the use of symptomatic preparations. If acute extrapyramidal symptoms develop, anticholinergic preparations should be prescribed. It is necessary to continue constant medical supervision and monitoring until the complete disappearance of signs of an overdose.
Side effects
The most commonly reported adverse reactions (incidence ≥ 10%) are parkinsonism, headache and insomnia.
Storage conditions
Store at a temperature not exceeding 30 ° C, out of the reach of children. Do not freeze.
