Sentor® tablets are used for the following indications:

treatment of essential arterial hypertension in adults and children over 6 years of age;
treatment of kidney disease in adult patients with arterial hypertension and type II diabetes mellitus with proteinuria ≥ 0.5 g / day - as part of blood pressure;
treatment of chronic heart failure (in patients aged 60 years and older), when the use of ACE inhibitors is considered impossible due to incompatibility, especially with cough or contraindicated; patients with heart failure who have stabilized with an ACE inhibitor should not be switched to treatment with losartan; the patient must have a left ventricular ejection fraction of ≤ 40%, be clinically stable, and follow an established chronic heart failure treatment regimen;
to reduce the risk of stroke in adult patients with arterial hypertension and left ventricular hypertrophy, as confirmed by ECG.

Compound

The active substance is losartan (one coated tablet contains 100 mg of losartan potassium).

Excipients: core: microcrystalline cellulose, pregelatinized starch, croscarmellose sodium, anhydrous colloidal silicon dioxide, magnesium stearate; Opadry II white shell (hypromellose, titanium dioxide (E 171), lactose monohydrate, macrogol, glycerol triacetate).

Contraindications

hypersensitivity to the active substance or any excipient that is part of the medicinal product;
severe liver dysfunction;
pregnancy or pregnancy planning;
simultaneous use with preparations that contain aliskiren is contraindicated in patients with diabetes mellitus or impaired renal function (glomerular filtration rate < 60 ml / minute / 1.73 m2).

Mode of application

The tablets should be taken with a glass of water. The use of the preparation does not depend on food intake.

The preparation for all indications is used 1 time per day at the same time.

Arterial hypertension

Usually the initial and maintenance dose for most patients is 50 mg (one tablet "Sentor®", 50 mg) 1 time per day. The maximum antihypertensive effect is achieved 3-6 weeks after the start of treatment. For some patients, the dose can be increased to 100 mg (one tablet "Sentor®", 100 mg) 1 time per day (in the morning).

The preparation can be used with other antihypertensive preparations, especially diuretics (for example, hydrochlorothiazide).

Patients with hypertension and type II diabetes mellitus with proteinuria ≥ 0.5 g/day

Usually the initial dose is 50 mg (one tablet "Sentor®", 50 mg) 1 time per day. The dose can be increased to 100 mg (one tablet "Sentor®", 100 mg) 1 time per day, depending on what the blood pressure readings are 1 month after the start of treatment. This medicinal product may be used with other antihypertensive agents (eg, diuretics, calcium channel blockers, α- or β-receptor blockers, centrally acting preparations), as well as with insulin and other commonly used hypoglycemic agents (eg, sulfonylurea, glitazones, and glucosidase inhibitors).

Heart failure

Usually the initial dose of losartan in patients with heart failure is 12.5 mg 1 time per day (the preparation should be used with the appropriate dosage). As a rule, the dose is increased at weekly intervals (for example, 12.5 mg, 25 mg, 50 mg, 100 mg and up to a maximum of 150 mg 1 time per day), taking into account the individual tolerability of the preparation.

Reducing the risk of stroke in patients with arterial hypertension and left ventricular hypertrophy confirmed by ECG

The usual starting dose is 50 mg of losartan (one Centor® 50 mg tablet) once daily. Depending on changes in blood pressure levels, low-dose hydrochlorothiazide should be added to treatment and / or the dose of losartan should be increased to 100 mg (one Centor® tablet, 100 mg) 1 time per day.

Application to children

Data on the efficacy and safety of the use of losartan in children over 6 years of age for the treatment of arterial hypertension are limited.

There is also little data on pharmacokinetics in children with arterial hypertension from the age of one month. For children who can swallow tablets and whose body weight is more than 20 kg and less than 50 kg, the recommended dose of losartan is 25 mg (1/2 tablet "Sentor®" 50 mg) 1 time per day. In exceptional cases, the dose can be increased to a maximum of 50 mg 1 time per day. The dose should be adjusted depending on the effect on blood pressure.

For patients weighing more than 50 kg, the usual dose is 50 mg 1 time per day. In exceptional cases, the dose can be increased to a maximum of 100 mg 1 time per day. Doses greater than 1.4 mg/kg (or 100 mg) per day have not been studied in children.

Losartan is not recommended for use in children under the age of 6 years, since data on the use of the preparation in this group of patients is not enough.

Losartan is not recommended for use in children with a glomerular filtration rate < 30 ml / minute / 1.73 m2, since there are no relevant data on the use of the preparation in this category of patients. Losartan is also not recommended for use in children with hepatic impairment.

Application features

Drivers

No studies have been conducted on the effect of the preparation on the ability to drive vehicles or other mechanisms. However, one should be aware of the possibility of developing such adverse reactions as dizziness and drowsiness, especially at the beginning of treatment and with an increase in the dose of the preparation.

Overdose

Symptoms of intoxication. Data on overdose in humans are limited. The most likely symptoms may be arterial hypotension and tachycardia. Bradycardia can occur with parasympathetic (vagal) stimulation.

Treatment of intoxication. If symptomatic hypotension occurs, supportive therapy should be carried out. Therapeutic measures depend on the length of time elapsed after taking the preparation, the nature and severity of the symptoms.

The priority measure should be the stabilization of the function of the cardiovascular system. After taking in case of overdose, the use of activated charcoal in the appropriate dose is indicated. Vital signs should be monitored frequently and adjusted as necessary. Losartan and active metabolites are not removed during hemodialysis.

Side effects

On the part of the blood and lymphatic system: frequent (from ≥ 1/100 to < 1/10) - anemia (patients with chronic heart failure).

From the side of the nervous system: frequent (from ≥ 1/100 to < 1/10) - muscle cramps (patients with arterial hypertension), dizziness (patients with arterial hypertension, patients with arterial hypertension and left ventricular hypertrophy, patients with chronic heart failure , patients with arterial hypertension and type II diabetes mellitus with renal insufficiency).

On the part of the hearing and labyrinth organs: frequent (from ≥ 1/100 to < 1/10) - vertigo (patients with arterial hypertension, patients with arterial hypertension and left ventricular hypertrophy of the heart).

From the side of the heart: frequent (from ≥ 1/100 to < 1/10) - tachycardia (patients with arterial hypertension).

From the vascular system: frequent (from ≥ 1/100 to < 1/10) - (orthostatic) hypotension (including dose-dependent orthostatic effect) (patients with chronic heart failure, patients with arterial hypertension and type II diabetes mellitus with renal failure ).

From the respiratory tract, chest and mediastinal organs: frequent (from ≥ 1/100 to < 1/10) - runny nose, sinusitis, pharyngitis, upper respiratory tract infection (patients with arterial hypertension).

From the digestive tract: frequent (from ≥ 1/100 to < 1/10) - dyspepsia (patients with arterial hypertension).

From the side of the kidneys and urinary tract: frequent (from ≥ 1/100 to < 1/10) - impaired renal function, renal failure (patients with chronic heart failure).
