    Product description 

    Sumilar capsules are indicated for the treatment of arterial hypertension in patients whose blood pressure is properly controlled by separate preparations, which are administered simultaneously in the same dose as in combination, but as separate tablets.

    Compound

    Active ingredient: amlodipine besilate, ramipril;
    1 hard capsule contains: 5 mg/5 mg 6.934 mg amlodipine besylate equivalent to 5 mg free base amlodipine and 5 mg ramipril or 10 mg/10 mg amlodipine besilate 13.868 mg equivalent to 10 mg free base amlodipine and 10 mg ramipril or 10 mg/day 5 mg 13.868 mg amlodipine besylate equivalent to 10 mg free base amlodipine and 5 mg ramipril, or 5 mg/10 mg amlodipine besylate 6.934 mg equivalent to 5 mg free base amlodipine and 10 mg ramipril;

    Excipients:

    Capsule contents: microcrystalline cellulose, anhydrous calcium hydrogen phosphate, corn starch, low moisture corn starch, sodium starch (type A), sodium stearyl fumarate;
    Capsule shell 10 mg/5 mg 5 mg/5 mg 5 mg/10 mg titanium dioxide (E 171), gelatin, iron oxide red (E172);
    Capsule shell 10 mg / 10 mg iron oxide yellow (E 172), iron oxide red (E172), iron oxide black (E172), titanium dioxide (E 171), gelatin.

    Contraindications

    Simultaneous use of preparations containing aliskiren, if the patient has diabetes or moderate or severe renal insufficiency (glomerular filtration rate (GFR) <60 ml / min / 1.73 m 2).
    History of angioedema (hereditary, idiopathic or previously transferred angioedema when taking ACE inhibitors or angiotensin II receptor antagonists).
    Extracorporeal treatment that results in blood coming into contact with negatively charged surfaces.
    Significant bilateral stenosis of the renal arteries or stenosis of the renal arteries of a single kidney.
    Pregnancy or pregnancy planning.
    Hypotensive or hemodynamically unstable condition.
    severe hypotension.
    Shock (including cardiogenic shock).
    Obstruction of the outflow tract from the left ventricle (for example, severe aortic stenosis).
    Hemodynamically unstable heart failure after acute myocardial infarction.
    Childhood.
    Hypersensitivity to amlodipine, dihydropyridine derivatives, ramipril or any other ACE inhibitors (ACE), or any excipients of the preparation.
    The combination of sacubitril/valsartan increases the development of angioedema.

    Mode of application

    The recommended daily dose is one capsule.
    Sumilar should be taken once a day, at the same time of day, with or without food. Capsules should be chewed or crushed.
    The fixed combination is not acceptable for initial therapy.
    If dose adjustment is necessary, you can change the dose of Sumilar or consider the possibility of individual titration of the components of the preparation. Capsules should be washed down with a sufficient amount of liquid (eg drinking water).

    Caution is advised, especially at the beginning of treatment and when changing the dosage.

    Overdose

    Symptoms associated with an overdose of ACE inhibitors may include excessive peripheral vasodilation (with severe arterial hypotension, shock), bradycardia, electrolyte imbalance and renal failure. The patient must be closely monitored. Treatment should be symptomatic and supportive. Supportive measures include primary detoxification (gastric lavage, absorbents) and measures to restore hemodynamic stability, in particular the use of alpha-1 adrenoceptor agonists or angiotensin II (angiotensinamide). Ramiprilat, the active metabolite of ramipril, is slightly removed from the general circulation by hemodialysis.

    Side effects

    Ramipril.
    The most common side effects of ramipril treatment are hyperkalemia, headache, dizziness, hypotension, orthostatic hypotension, syncope, cough (unproductive cough), bronchitis, sinusitis, shortness of breath, inflammation of the gastrointestinal tract, indigestion, abdominal pain, dyspepsia, diarrhea , nausea, vomiting, rash, in particular maculopapular, muscle cramps, myalgia, chest pain, fatigue. Serious side effects include agranulocytosis, pancytopenia, myocardial infarction, angioedema, vasculitis, bronchospasm, acute pancreatitis, liver failure, acute renal failure, hepatitis, exfoliative dermatitis, toxic epidermal necrolysis, Stevens-Johnson syndrome, and erythema multiforme.
    Amlodipine.
    The most common side effects of amlodipine treatment are drowsiness, dizziness, headache, heart palpitations, hot flashes, abdominal pain, nausea, ankle swelling, edema, and fatigue. Serious side effects include leukopenia, thrombocytopenia, myocardial infarction, atrial fibrillation, ventricular tachycardia, vasculitis, acute pancreatitis, hepatitis, angioedema, erythema multiforme, exfoliative dermatitis, and Stevens-Johnson syndrome.

    Interaction

    The simultaneous use of preparations containing aliskiren is contraindicated if the patient has diabetes or moderate or severe renal insufficiency (glomerular filtration rate (GFR) <60 ml / min / 1.73 m 2).

    Storage conditions
