Tablets "Telmista H 40" are used for arterial hypertension. The preparation in a fixed dose combination (40 mg telmisartan / 12.5 mg hydrochlorothiazide) is indicated for use in adult patients if telmisartan monotherapy does not provide adequate blood pressure control.
One tablet contains (active ingredients):
Telmisartan - 40 mg;
hydrochlorothiazide - 12.5 mg.
Excipients: meglumine, sodium hydroxide, povidone, lactose polyhydrate, sorbitol (E 420), magnesium stearate, mannitol (E 421), mannit DC (E 421), hydroxypropyl cellulose, anhydrous colloidal silicon dioxide, sodium stearyl fumarate, iron oxide red (E 172).
The preparation is prescribed to patients in whom blood pressure is insufficiently controlled when using Telmisartan alone. Before switching to preparation treatment, the dose of each of the components should be determined. Direct replacement of monotherapy with fixed combinations may be considered.
The preparation "Telmista H 40" can be prescribed once a day to patients whose blood pressure is insufficiently controlled when using Telmisartan 80 mg tablets.
The preparation should be taken orally once a day with a liquid, regardless of food intake.
Drivers
When driving a car and working with mechanisms, it should be borne in mind that during antihypertensive therapy with the preparation, dizziness or drowsiness may occur.
Information on overdose with telmisartan in humans is limited. The extent to which hydrochlorothiazide is removed by hemodialysis is unknown.
Symptoms
The most likely expected manifestations of an overdose of telmisartan are arterial hypotension and tachycardia; bradycardia, dizziness, vomiting, increased serum creatinine and acute renal failure were also observed. An overdose of hydrochlorothiazide is associated with a decrease in electrolytes (hypokalemia, hypochloremia) and dehydration due to excessive diuresis. The most common signs and symptoms of an overdose are nausea and drowsiness. Hypokalemia can lead to muscle spasm and / or exacerbation of cardiac arrhythmias, which are associated with the simultaneous use of digitalis preparations or certain antiarrhythmic preparations.
Treatment
Telmisartan is not removed by hemodialysis. Patients should be closely monitored. Treatment for overdose symptoms should be symptomatic and supportive. Therapy depends on the timing of the pill and the severity of the symptoms. Supportive measures include induction of vomiting and / or gastric lavage. In the treatment of overdose, the use of activated charcoal may be helpful. Electrolyte and creatinine levels should be monitored. If arterial hypotension occurs, the patient should be placed on his back and treated with saline and bulk solutions.
An adverse reaction that was frequently observed was dizziness.
