Telpres tablets are used for the following indications:
The active ingredient is telmisartan (one tablet contains 40 mg telmisartan).
Excipients: sodium hydroxide, povidone, meglumine, lures (E 421), crospovidone, magnesium stearate.
Telpres is taken orally once a day with a sufficient amount of liquid, regardless of food intake.
Telpres should be stored in a sealed blister pack, as tablets are very hygroscopic. Remove the tablets from the blister immediately before use.
Essential hypertension treatment
The usual effective dose is 40 mg per day. For some patients, a daily dose of 20 mg may be sufficient. If the desired blood pressure is not reached, the dose of telmisartan can be increased to 80 mg once a day. Alternatively, telmisartan can be given in combination with a thiazide diuretic such as hydrochlorothiazide, which has been shown to further lower blood pressure when used with telmisartan. When considering increasing the dose, it should be borne in mind that the maximum antihypertensive effect is generally achieved after 4-8 weeks from the start of treatment.
Prevention of cardiovascular diseases
The recommended dose is 80 mg once a day. The efficacy of telmisartan in doses less than 80 mg in the prevention of cardiovascular disease is unknown.
When starting treatment with telmisartan to prevent cardiovascular disease, it is recommended to monitor blood pressure and, if necessary, adjust the dose of preparations that lower blood pressure.
Drivers
With antihypertensive therapy, including telmisartan, when driving vehicles or other mechanisms, it is necessary to take into account the possibility of dizziness or hypersomnia.
Information on overdose with telmisartan is limited.
Symptoms
The most pronounced manifestations of telmisartan overdose were arterial hypotension and tachycardia; there were also cases of bradycardia, dizziness, increased serum creatinine and acute renal failure.
Treatment
Telmisartan is not removed from the body by hemodialysis. In case of an overdose, the patient should be closely monitored and symptomatic and supportive therapy prescribed. The choice of treatment depends on the time that has elapsed after administration and on the severity of the symptoms. Consideration should be given to measures such as inducing vomiting and / or gastric lavage. Activated charcoal can be helpful in treating overdose. Serum electrolyte and creatinine levels should be monitored frequently. When arterial hypotension occurs, the patient should be placed in a supine position and quickly introduce salt substitutes and restore the volume of fluid.
Serious adverse reactions, including anaphylactic reactions and angioedema, are rare (≥ 1/10 000 to <1/1000), and acute renal failure has also been observed.
