Posterisan Forte is a high-potency proctological preparation designed to provide rapid relief during acute flare-ups of hemorrhoidal disease and severe inflammatory conditions of the perianal area. Unlike the standard version, the Forte formulation combines the immunomodulatory benefits of bacterial lysates with the powerful anti-inflammatory properties of hydrocortisone. This dual-action approach is specifically engineered to target intense itching, significant swelling, and acute pain that often accompany advanced hemorrhoids or severe anal eczema. By simultaneously suppressing excessive inflammation and stimulating local tissue immunity, Posterisan Forte helps stabilize the affected area, allowing for a faster transition to the healing phase while providing immediate symptomatic comfort.
Each 1 gram of Posterisan Forte ointment contains:
Active Substances: 166.7 mg of a standardized suspension of bacterial culture (CSBC), containing at least 500 million inactivated Escherichia coli bacteria (including cell wall components and metabolic products) and 2.5 mg of Hydrocortisone.
Preservative: 3.3 mg of Liquefied Phenol.
Excipients: Lanolin (wool fat), white soft paraffin (petrolatum), and antioxidants.

Anti-Inflammatory & Antipruritic: Hydrocortisone, a low-potency corticosteroid, works rapidly to reduce capillary permeability, stabilize lysosomal membranes, and suppress the release of inflammatory mediators. This leads to a significant reduction in redness, swelling, and the burning sensation.
Immunomodulatory Effect: The bacterial lysates (E. coli) act as a local vaccine, stimulating macrophages and T-lymphocytes within the anal mucosa. This increases the production of Secretory Immunoglobulin A (sIgA) and enhances the resistance of the tissue to secondary infections.

Because this product contains a corticosteroid, it is designed for short-term use. Treatment should generally not exceed 10 days. Once the acute symptoms have subsided, it is often recommended to switch to the regular (non-steroid) Posterisan ointment to maintain local immunity and complete the healing process.
When used according to instructions, side effects are rare, but may include:
Local Reactions: Mild burning, stinging, or allergic contact dermatitis (often due to lanolin or phenol).
Steroid-Related: If used for more than 2-3 weeks, there is a risk of skin atrophy (thinning of the skin), telangiectasia (visible small blood vessels), or secondary infections.
