Description
Indications.
Seroquel XR is indicated for the treatment of:
Schizophrenia, including prevention of relapse in patients with stable schizophrenia who received maintenance therapy with Seroquel XR.
Bipolar disorder, in particular:
for the treatment of moderate and severe manic episodes in bipolar disorder;
for the treatment of severe depressive episodes in bipolar disorder;
for the prevention of relapse of the disease in patients with bipolar disorder, in patients with manic or depressive episodes, in which treatment with Seroquel XR is effective.
As adjunctive therapy for major depressive episodes in patients with major depressive disorder (SID) who have a suboptimal response to antidepressant monotherapy. The safety profile of Seroquel XR should be carefully considered by the physician prior to initiating therapy.
Contraindications.
Hypersensitivity to the active substance or to any component of the preparation.
Concomitant use of cytochrome P450 3A4 inhibitors such as HIV protease inhibitors, azole antifungals, erythromycin, clarithromycin and nefazodone is contraindicated.
Compound

50 mg tablets 1 tablet contains 57.56 mg of quetiapine fumarate, which corresponds to 50 mg of quetiapine;
200 mg tablets 1 tablet contains 230.26 mg of quetiapine fumarate, which corresponds to 200 mg of quetiapine;
300 mg tablets 1 tablet contains 345.38 mg of quetiapine fumarate, which corresponds to 300 mg of quetiapine;
400 mg tablets 1 tablet contains 460.50 mg of quetiapine fumarate, which corresponds to 400 mg of quetiapine;
Excipients:
lactose monohydrate, sodium microcrystalline cellulose; magnesium stearate hypromellose 2208; hypromellose 2910; polyethylene glycol 400; titanium dioxide (E 171)
for tablets of 50, 200 and 300 mg - iron oxide yellow (E172)
for tablets of 50 mg - iron oxide red (E 172).
Method of application and dose.
There are different dosing schedules for each indication. It should be ensured that the patient is prescribed the dosage according to his condition.
Seroquel XR should be taken once daily on an empty stomach. Tablets should be swallowed whole, without breaking, chewing or crushing them.
For the treatment of schizophrenia and moderate to severe manic episodes in bipolar disorder
Seroquel XR should be taken at least 1 hour before meals. The daily dose at the beginning of therapy is 300 mg on the first day and 600 mg on the second day. The recommended daily dose is 600 mg, but if clinically justified, the dose can be increased to 800 mg per day. The dose should be adjusted within the range of effective doses - from 400 mg to 800 mg per day - depending on the clinical response and tolerability. For maintenance therapy in schizophrenia, no dose adjustment is necessary.
For the treatment of depressive episodes in bipolar disorder
Seroquel XR should be taken at bedtime. The total daily dose for the first four days of treatment is 50 mg (day 1), 100 mg (day 2), 200 mg (day 3) and 300 mg (day 4). The recommended daily dose is 300 mg. In clinical studies, no additional benefit was observed in the 600 mg group compared to the 300 mg group (see section "Pharmacological properties"). A dose of 600 mg may be effective in some patients. Doses above 300 mg should be prescribed by a physician experienced in the treatment of bipolar disorder. Clinical studies show that for individual patients in the event of problems associated with preparation intolerance, dose reduction to a minimum of 200 mg should be considered.
To prevent relapse in bipolar disorder
To prevent further manic, mixed or depressive episodes in bipolar disorder, patients who have responded to Seroquel XR in the emergency treatment of bipolar disorder should continue treatment with Seroquel XR at the same prescribed dose at bedtime. The dose of Seroquel XR may be adjusted within the dose range of 300 mg to 800 mg/day, depending on the clinical response and tolerability of each individual patient. It is important that the lowest effective doses be used for maintenance therapy.
For concomitant treatment of major depressive episodes in TIR
Seroquel XR should be taken at bedtime. The daily dose at the beginning of therapy is 50 mg on the 1st and 2nd day and 150 mg on the 3rd and 4th day. In short-term studies of concomitant therapy (with amitriptyline, bupropion, citalopram, duloxetine, escitalopram, fluoxetine, paroxetine, sertraline and venlafaxine), an antidepressant effect was observed at doses of 150 and 300 mg/day and at a dose of 50 mg/day in a short-term study of monotherapy . When using high doses of the preparation, the risk of developing adverse reactions increases. Therefore, the physician should ensure that the lowest effective dose, starting at 50 mg/day, is used for treatment. The need to increase the dose from 150 to 300 mg/day should be based on an assessment of the individual patient's condition.
Transfer from the preparation Seroquel, tablets with immediate release of the active substance
For more convenient dosage selection for patients treated with individual doses of Seroquel (tablets with immediate release of the active substance), it is possible to convert to the use of the preparation Seroquel XR in an equivalent total daily dose of 1 time per day. It may be necessary to individually adjust the dose.
Elderly patients
As with other antipsychotics and antidepressants, Seroquel XR should be used with caution in elderly patients, especially during the initial dose selection period. A slower dose titration of Seroquel XR may be required, and the daily therapeutic dose may be lower than that used in younger patients. The mean clearance of quetiapine was 30% to 50% lower in elderly patients compared to younger patients. Treatment of elderly patients should begin with 50 mg / day. The dose may be increased by 50 mg/day to an effective dose depending on clinical response and tolerability in the individual patient.
In elderly patients with HCV major depressive episodes, treatment should be initiated at 50 mg/day on days 1 to 3, increased to 100 mg/day on day 4 and 150 mg/day on day 8. The lowest effective dose should be used. starting at 50 mg/day. If it is necessary to increase the dose to 300 mg / day, based on an assessment of the condition of a particular patient, this is possible no earlier than day 22.
The safety and efficacy of use in patients over 65 years of age with depressive episodes associated with bipolar disorder have not been studied.
