Pharmacological properties
Pharmacodynamics. Aminoven Infant 10% amino acids included in the preparation are physiological components. after parenteral administration, they are included in the pool of free amino acids in the body and are involved in all metabolic processes, in particular, they are used for protein synthesis.
Pharmacokinetics. Bioavailability of the preparation Aminoven Infant 10% with intravenous administration is 100%. Amino acids are included in the total pool of free amino acids in the body and are distributed in the interstitial fluid and intercellular space of organs and tissues. The concentration of free amino acids in blood plasma and cell cytoplasm is regulated within narrow limits depending on the age, diet and general condition of the patient. When administered correctly (slowly and at a constant rate), Aminoven Infant 10% does not disturb the amino acid balance. In severe disorders of liver and kidney function, the regulation of the balance of amino acids is disrupted. In these cases, other formulations of amino acid solutions for parenteral nutrition should be used. Only a small proportion of amino acids are eliminated by the kidneys. T½ of amino acids from blood plasma is highly dependent on age.
Indications
Aminoven Infant 10% is intended for partial parenteral nutrition of premature babies, newborns and young children. combined with solutions of carbohydrates, fat emulsions, as well as preparations of vitamins, electrolytes and microelements provides complete parenteral nutrition.
Application
Aminoven Infant 10% is intended for long-term intravenous drip, mainly in the central veins.
Maximum rate of administration: 0.1 g of amino acids per 1 kg of body weight per hour, which is equal to 1 ml / kg of body weight per hour.
Maximum daily dose:
children under the age of 1 year - 1.5–2.5 g of amino acids per 1 kg of body weight per day or 15–25 ml of solution Aminoven Infant 10% per 1 kg of body weight per day;
children aged 2–5 years - 1.5 g of amino acids per 1 kg of body weight per day or 15 ml / kg of body weight per day;
children aged 6-14 years - 1.0 g of amino acids per 1 kg of body weight per day or 10 ml / kg of body weight per day.
Aminoven Infant 10% is used as long as there is a need for parenteral nutrition.
Contraindications
Amino acid metabolism disorders, metabolic acidosis, hyperhydration, hyperkalemia, hypersensitivity to the preparation, shock, hypoxia, decompensated heart failure, renal failure in the absence of hemodialysis or hemofiltration, severe liver failure. with hepatic and renal failure, individual dosage is required. use with caution in patients with hyponatremia.
Side effects
When used correctly, they have not been identified. development of allergic reactions is possible. with the introduction of the preparation Aminoven Infant 10% into the peripheral veins, local reactions may occur: hyperemia, phlebitis, thrombosis. daily monitoring of the puncture site is recommended.
Special instructions
With parenteral nutrition of young children, it is necessary to take into account the following indicators: urine nitrogen, content of ammonia, glucose, electrolytes, tg (with additional introduction of fat emulsions), liver enzymes, serum osmolarity, acid-base balance and water-salt metabolism.
Too rapid infusion can lead to the loss of amino acids through the kidneys and thus to an imbalance of amino acids.
Incompatibility. Due to the increased risk of microbial contamination, amino acid solutions should not be mixed with other preparations.
It is necessary to avoid the addition of other medicines to the preparation Aminoven Infant 10%, as this can cause toxic reactions. In any case, you should ensure that the preparations are compatible, maintain sterility and mix thoroughly. Solutions with the addition of other preparations cannot be stored.
Children. Aminoven Infant 10% is intended for partial parenteral nutrition of premature babies, newborns and young children.
Influence on the ability to drive vehicles or work with other mechanisms. Have not studied.
Interactions
Currently, there are no known cases of interaction of Aminoven Infant 10% with other preparations.
Overdose
When the dosage or rate of administration of the preparation is exceeded, Aminoven Infant 10%, as in the case of an overdose of other solutions of amino acids, there are chills, nausea, vomiting, renal aminoacidosis. in this case, the administration of the preparation should be stopped immediately. when hyperkalemia appears, inject from 200 to 500 ml of 5% dextrose (glucose) solution with the addition of 1-3 IU of insulin for every 3-5 g of dextrose (glucose). it is possible to continue therapy with the preparation using low doses.
Storage conditions
In a dark place at a temperature not exceeding 25 ° C. do not freeze. shelf life is 2 years.
