Pharmacological properties

Pharmacodynamics. Asparcam has antiarrhythmic properties, replenishes the deficiency of magnesium and potassium ions. magnesium activates the sodium-potassium atmosphere, as a result of which the intracellular sodium content decreases and the entry of potassium into the cell increases. with a decrease in sodium concentration in the cell, sodium-calcium metabolism in vascular smooth muscles is inhibited, which leads to their relaxation, potassium ions stimulate the synthesis of ATP, glycogen, proteins, acetylcholine; potassium and magnesium support the polarization of cell membranes. aspartate promotes the entry of ions into the cell and participates in energy metabolism. the antiarrhythmic effect is realized due to the ability of the preparation to eliminate electrolyte imbalance, reduce the excitability and conductivity of the myocardium.

Pharmacokinetics. Has not been investigated.

Indications

For additional therapy for chronic heart diseases (with heart failure, in the postinfarction period, with cardiac arrhythmias, especially with ventricular arrhythmias); additional therapy in the treatment of digitalis preparations.

Application

The preparation is intended for intravenous administration only. for adults, inject slowly intravenously drip - 10–20 ml each (dilute the contents of the ampoule in 50–100 ml of 5% dextrose solution). if necessary, the dose can be re-administered after 4–6 hours. The preparation is suitable for combination therapy.

The course of treatment is determined by the doctor.

Children. The experience of using the preparation in children is insufficient, therefore it is not used in this category of patients.

Contraindications

Hypersensitivity to the components of the preparation; acute and chronic renal failure; Addison's disease av blockade III degree; cardiogenic shock (hell 90 mm Hg).

Side effects

With rapid intravenous administration of the preparation, the possibility of the appearance of symptoms of hyperkalemia, for example, nausea, vomiting, diarrhea, paresthesia, and / or hypermagnesemia, for example, facial flushing, hyporeflexia, convulsions, fever, respiratory depression should be considered.

Special instructions

With prolonged use of the preparation, it is necessary to control the level of potassium and magnesium in the blood, and also requires regular monitoring of electrolyte hemostasis and ECG data.

Asparkam, as a preparation containing potassium and magnesium, should be used with caution in patients with myasthenia gravis; in conditions that can lead to hyperkalemia, such as acute dehydration, widespread tissue damage, in particular in severe burns. In this category of patients, it is recommended to regularly monitor the level of electrolytes in the blood serum.

Since the preparation contains sorbitol, patients with hereditary fructose intolerance should not use this preparation.

Use during pregnancy and lactation. So far, it has not been reported about the occurrence of any danger when using the preparationin this category of patients.

Interactions

The simultaneous use of the preparation with potassium-sparing diuretics and / or APF inhibitors increases the risk of hyperkalemia.

Overdose

Overdose cases are unknown. in case of overdose, symptoms of hyperkalemia and hypermagnesemia can be observed.

Symptoms of hyperkalemia: general weakness, paresthesia, bradycardia, paralysis. An extremely high concentration of potassium in the blood plasma can be fatal from cardiac depression, arrhythmias, or cardiac arrest.

Symptoms of hypermagnesemia: nausea, vomiting, drowsiness, hypotension, bradycardia, weakness, slurred speech, double vision. At very high plasma concentrations of magnesium, hyporeflexia, muscle paralysis, respiratory arrest and cardiac arrest can develop.

In case of overdose, K + -, Mg2 + -aspartate should be canceled and symptomatic treatment is recommended (calcium chloride 100 mg / min IV, dialysis, if necessary).
