Pharmacological properties

Pharmacodynamics. Exoderil® nail polish, 50 mg / ml nail polish is a topical antifungal agent. amorolfine belongs to a new class of chemical compounds, its fungicidal effect is to change the membrane of the fungal cell, mainly due to the effect on sterol biosynthesis. the content of ergosterol decreases and at the same time there is an accumulation of atypical steric non-planar sterols.

Amorolfine is a broad-spectrum antimycotic. It is highly active (MIC 2 μg / ml) in vitro against:

dermatophytes Trichophyton, Microsporum, Epidermophyton;

yeast Candida, Cryptococcus, Malassezia;

molds Hendersonula, Alternaria, Scopulariopsis, Scytalidium, Aspergillus;

fungi of the family Dematiaceae Cladosporium, Fonsecaea, Wangiella;

dimorphic fungi Coccidioides, Histoplasma, Sporothrix.

Bacteria, with the exception of actinomycetes, are insensitive to the action of amorolfine.

Pharmacokinetics. Amorolfine from nail polish penetrates into the nail plate and diffuses through it and therefore is able to eradicate fungi that are in the nail bed and which are difficult to reach. The systemic absorption of the active ingredient with this application is very low.

There is no evidence of accumulation of the drug in the body after prolonged use of the drug Exoderil varnish, nail varnish at 50 mg / ml.

Indications

Treatment for onychomycosis without damage to the matrix (mild and moderate) caused by dermatophytes, yeasts and molds.

Application

Nail polish should be applied to the affected area of ​​the finger or nail once a week.

The patient should apply the varnish as follows:

1. Before the first application of amorolfine 5%, nail polish, it is very important that the affected areas of the nail (in particular the surface of the nail) are sanded as thoroughly as possible using the special nail file scraper that is included. Then the surface should be cleaned and degreased with an alcohol wipe. Before reapplying amorolfine 5%, the affected area of ​​the nail should be sanded again, after which the nail should be cleaned with an alcohol wipe.

Warning: File scrapers that have been used on affected nails should not be used on healthy nails.

2. Using one of the reusable applicators, apply varnish to the entire surface of the affected nail. Allow the varnish to dry for 3-5 minutes. After use, clean the applicator with the same alcohol wipe as the nail. Keep bottle tightly closed.

Applying the preparation to each nail, immerse the applicator in a bottle of varnish without wiping the applicator with varnish on the neck of the bottle.

Warning: when working with organic solutions (solvents, mineral spirits, etc.), waterproof gloves should be used to keep the varnish on the nails.

Treatment should be continued without interruption until the nail heals and the affected areas heal. The required frequency and duration of treatment depends mainly on the intensity and location of the infection. This usually lasts 6 months (for fingernails) and 9-12 months (for toenails). It is recommended to repeat the treatment at intervals of about 3 months.

If there is also dermophytosis of the foot, it is necessary to apply an appropriate antifungal cream.

Contraindications

Hypersensitivity to amorolfine or excipients that make up the drug.

Side effects

Adverse drug reactions are rare. possible damage to the nails (for example, discoloration of the nails, brittleness, fragility of the nails). these reactions can also be caused by onychomycosis itself.

On the part of the skin and subcutaneous tissues: rarely (≥1 / 10,000, 1/1000): violation of the nails, discoloration of the nails, onychoclasia (brittle nails), onychorexis (brittle nails); very rare (1/10 000): burning sensation of the skin.

Special instructions

The use of nail polish and false nails should be avoided during drug treatment.

Avoid contact with eyes, ears and mucous membranes.

Patients with underlying medical conditions that make them prone to nail fungal infections (peripheral circulation disorders, diabetes mellitus and immune suppression) should consult a doctor.

Patients with nail dystrophy and a destroyed nail plate should consult a doctor.

Use during pregnancy and lactation. In studies of reproductive toxicity in laboratory animals, no signs of teratogenicity were detected, but with oral administration of the drug in high doses, an embryotoxic effect was noted. Systemic absorption of amorolfine during and after topical application is very low, therefore the risk to the human fetus is very low. However, since there is no relevant experience in use, the use of Exoderil varnish should be avoided during pregnancy and lactation.

Children. The safety and efficacy of the drug in children (under the age of 18) has not been established at present.

Interactions

There are no special studies on the combined treatment with other drugs for external use.

Overdose
