Product description

Lipster tablets are used:

• For the treatment of infections of the skin and mucous membranes caused by the Herpessimplex virus, including primary infections of genital herpes and relapses (with the exception of neonatal infections of herpes simplex and severe infections caused by Herpes simplex in children with an impaired immune response).
• For the prevention of recurrent infections with Herpes simplex in patients with normal immunity.
• For the prevention of Herpes simplex infection in immunocompromised patients.
• For the treatment of infections caused by the Varicella zoster virus (chickenpox and herpes zoster).

Composition

Active ingredient: acyclovir.

1 tablet contains acyclovir in terms of 100% anhydrous substance 200 mg or 400 mg, or 800 mg.

Excipients: microcrystalline cellulose, povidone, sodium starch (type A), magnesium stearate.

Contraindications

Hypersensitivity to acyclovir, valacyclovir or other components of the preparation.

Mode of application

The tablets should be taken whole with water.

Adults: 1 tablet 200 mg 5 times a day every 4 hours, except during the night period. Treatment should be continued for 5 days; in case of severe primary infection, it can be extended. The first dose should be taken as soon as possible in the event of an infection or relapse - at the first sign of infection or immediately after blisters appear.

Application features

Hydration: Care should be taken to ensure that patients receiving large doses of oral or parenteral acyclovir receive adequate fluid.

The use of other nephrotoxic preparations increases the risk of renal failure.

Use in patients with renal insufficiency and in elderly patients, acyclovir is excreted by the kidneys, and therefore, for patients with renal insufficiency, the dose should be reduced. It should be borne in mind that in the elderly, the likelihood of impaired renal function is high, therefore, a dose reduction may also be required for this category. Elderly patients and patients with renal insufficiency have an increased risk of developing neurological adverse reactions, therefore, careful monitoring of the occurrence of these adverse reactions is necessary. In reported cases, these reactions were usually reversible upon discontinuation of treatment.

In patients with severe immune deficiency, long-term treatment with acyclovir or periodic repeated cycles of treatment with acyclovir can lead to the appearance of viral strains with reduced sensitivity to acyclovir. In this case, continuing treatment with acyclovir may not be effective.

The available clinical trial data are not sufficient to conclude that acyclovir treatment reduces the incidence of chickenpox-related complications in immunocompetent patients.

Pregnant

Acyclovir should be used only when the potential benefit of the preparation to a pregnant woman outweighs the potential risk to the fetus.

Children

Acyclovir tablets should be used in children aged 2 years and older.

Drivers

When considering the possibility of driving a car and other mechanisms, one should take into account the clinical status of the patient and the profile of side effects of the preparation. Clinical studies of the effect of acyclovir on the reaction rate when driving or working with other mechanisms have not been conducted. In addition, the pharmacology of acyclovir does not give reason to expect any negative effects.

Overdose

Acyclovir is only partially absorbed from the gastrointestinal tract. There are known cases of patients unintentionally taking up to 20 g of acyclovir without causing a toxic effect. Accidental repeated overdose of acyclovir over several days causes gastroenterological (such as nausea and vomiting) and neurological symptoms (headache and confusion).

In case of an overdose of intravenous acyclovir, the level of serum creatinine, blood urea nitrogen increases, and thus renal failure appears. Overdose neurological manifestations can be confusion, hallucinations, agitation, convulsions, and coma.

Side effects

On the part of the blood and lymphatic system

Very rare: anemia, leukopenia, thrombocytopenia.

From the immune

systems: rarely anaphylaxis.

Mental and nervous system disorders

Often: headache, dizziness.

Very rare: agitation, confusion, tremor, ataxia, dysarthria, hallucinations, psychotic symptoms, convulsions, drowsiness, encephalopathy, coma.

Interaction

Acyclovir is excreted mainly unchanged by the kidneys by tubular secretion, so any preparations that have a similar release mechanism can increase the concentration of acyclovir in plasma.

Probenecid and cimetidine increase the half-life of acyclovir and increase the AUC of acyclovir.

With the simultaneous use of acyclovir with mycophenolate mofetil, an immunosuppressive agent, which is used in patients after transplantation, an increase in the AUC of acyclovir and the inactive metabolite of mycophenolate mofetil was noted. But given the broad therapeutic index of acyclovir, there is no need to change the dose.

In an experimental study of the simultaneous use of acyclovir and theophylline in 5 people, an increase in the AUC of the total dose of theophylline was found by about 50%. It is recommended to control the content of theophylline in plasma while treating with acyclovir.
