Pharmacological properties

Pharmacodynamics. The active substance of the preparation (mebikar) is close in chemical structure to the natural metabolites of the body: the mebicar molecule consists of two methylated urea fragments that make up the bicyclic structure.

Mebikar IC has a moderate tranquilizing (anxiolytic) activity, eliminates or weakens the feeling of anxiety, anxiety, fear, internal emotional stress and irritation. The sedative effect of the preparation is not accompanied by muscle relaxation and impaired coordination of movements. The preparation reduces mental and physical activity, so it can be used during the working day or during training. The preparation does not create high spirits, feelings of euphoria. On this basis, Mebikar IC is referred to as daytime tranquilizers. It enhances the effect of hypnotics and improves sleep when it is disturbed, but does not have a hypnotic effect.

In addition to the sedative, the preparation has a nootropic effect. The preparation improves cognitive functions, increases attention and mental performance, without stimulating the symptoms of productive psychopathological disorders, such as delirium, pathological emotional activity.

The preparation has antioxidant activity, therefore it acts as a membrane stabilizer, adaptogen and cerebroprotector in oxidative stress of various origins.

The preparation acts on the activity of the structures of the limbic-reticular complex, in particular on the emotiogenic zones of the hypothalamus, and also affects all four main neurotransmitter systems - GABAergic, cholinergic, serotonergic and adrenergic, which contributes to their balance and integration, but does not have a peripheral adrenonegative effect ... A dopamine-positive component is present in the neurotransmitter profile of the preparation. The preparation exhibits antagonistic activity in relation to the excitation of the adrenergic and glutamatergic systems and enhances the functioning of the inhibitory serotonin- and GABA-ergic mechanisms of the brain. The preparation has normosthenic properties.

Pharmacokinetics. When administered orally, bioavailability is 77–80%; up to 40% of the preparation binds to erythrocytes; the remaining 60% do not bind to blood proteins and are contained in the blood plasma in a free state, and therefore the preparation is widely distributed in the body and penetrates the membranes. Cmax in blood plasma is reached in 30 minutes, a high level remains for 3-4 hours, then gradually decreases. The preparation is completely excreted from the body with urine within 1 day, does not accumulate and is not subject to biochemical transformations in the body.

Indications

Neuroses and neurosis-like states, accompanied by irritation, emotional lability, feelings of anxiety and fear; to improve the tolerance of neuroleptics or tranquilizers in order to eliminate the somatovegetative and neurological side effects caused by them; cardialgia of various origins (not associated with ischemic heart disease); to facilitate the course of somatovegetative manifestations in premenstrual syndrome and menopause.

The preparation is indicated as a cerebroprotective agent and an adaptogen for emotional and oxidative stress of various origins.

As part of the complex therapy of nicotine addiction as a means of reducing the craving for smoking.

Application

The preparation is taken orally, regardless of food intake. the dosage depends on the individual characteristics of the patient and the therapeutic purpose of the preparation.

Tablets 300 mg. The usual dose is 300-600 mg 2-3 times a day. The dose can be increased if necessary. The maximum single dose is 3 g, the maximum daily dose is 10 g. The duration of the course of treatment is from several days to 2-3 months. As part of the complex treatment of nicotine addiction, the preparation is used at 600-900 mg 3 times a day for 5-6 weeks.

500 mg tablets. The usual adult dose is 500 mg 2-3 times a day, the dose may be increased if necessary. The maximum single dose is 3 g, the daily dose is 10 g. The duration of the course of treatment is from several days to 2-3 months.

When used as a means that weakening dependence on smoking, the preparation is prescribed 500-1000 mg 3 times a day for 5-6 weeks.

If one or more doses of the preparation are missed, it is necessary to continue the course of treatment according to the previously prescribed scheme.

Contraindications

Individual sensitivity to preparation components.

Side effects

The preparation is usually well tolerated.

From the side of the cardiovascular system: rarely - lowering blood pressure.

From the digestive tract: rarely - dyspeptic disorders (including nausea, vomiting). In this case, the dose of the preparation should be reduced.

From the respiratory system: rarely - bronchospasm.

On the part of the skin and subcutaneous tissue: rarely - allergic reactions (including skin rash, itching, urticaria, angioedema). In case of an allergic reaction, the preparation is discontinued.

General disorders: rarely - a decrease in body temperature, weakness, dizziness. With a decrease in blood pressure and / or body temperature (body temperature may decrease by 1-1.5 ° C), you should not stop taking the preparation. Blood pressure and body temperature normalize on their own.

Special instructions

It should be used with caution in case of impaired liver and kidney function, in patients with arterial hypotension. addiction, dependence and withdrawal syndrome with the use of the preparation have not been established.

Use during pregnancy and lactation. Mebikar IC penetrates well into all tissues and body fluids. The preparation should not be used during pregnancy and lactation due to the lack of sufficient clinical data regarding the use of the preparation in this group of patients.

Children. The preparation is not intended for use in children.

The ability to influence the reaction rate when driving or working with other mechanisms. Care should be taken when driving vehicles, since sometimes the use of the preparation can lead to a decrease in blood pressure.

Interactions

Mebikar ic can be combined with antipsychotics, tranquilizers (benzodiazepines), hypnotics, antidepressants and psychostimulants.

Overdose

The preparation is low toxic. with a significant overdose, side effects may increase (including allergic reactions, dyspeptic disorders, weakness, a temporary decrease in blood pressure and body temperature).

Treatment: it is necessary to carry out gastric lavage and conventional detoxification methods.
