Pharmacological properties
Pharmacodynamics. xylometazoline is a sympathomimetic agent (imidazole derivative) that acts on the α-adrenergic receptors of the nasal mucosa. after nasal administration, xylometazoline causes vasoconstriction, thereby reducing swelling of the nasopharyngeal mucosa.
The action of the preparation begins within 2 minutes after application and lasts up to 12 hours.
The preparation is well tolerated, including by patients with sensitive mucous membranes. The preparation reduces mucociliary function. The results of laboratory studies have shown that xylometazoline reduces the infectious activity of human rhinovirus, which is associated with common ARVI.
Pharmacokinetics. After nasal administration, the amount of absorbed substance can sometimes be sufficient to obtain systemic effects, for example, on the central nervous system and the cardiovascular system.
No metabolites and routes of their excretion have been identified.
Indications
- Symptomatic treatment of nasal congestion with colds, hay fever, sinusitis, allergic rhinitis;
- to facilitate the outflow of secretions from the paranasal sinuses;
- as an auxiliary therapy for otitis media to eliminate edema of the nasopharyngeal mucosa;
- to facilitate rhinoscopy.
Application
Multigrip nasal, nasal drops, solution 0.05%. children aged 2–12 years. the recommended dose is 1-2 drops 1-2 times a day in each nasal passage.
Multigrip Nasal, nasal drops, solution 0.1%. Adults and children over the age of 12. The recommended dose is 2-3 drops 2-3 times a day in each nasal passage.
Do not use in doses exceeding the recommended ones.
Duration of use. The preparation should not be used for more than 5 days in a row, unless otherwise recommended by your doctor.
Before further use of xylometazoline, you should take a break for several days between doses.
Multigrip Nasal, nasal spray, solution 0.05%. Children from 2 to 12 years old. 1 injection into each nasal passage, if necessary, but no more than 3 times a day.
Multigrip Nasal, nasal spray, solution 0.1%. Adults and children over the age of 12. The recommended dose is 1 injection into each nasal passage 3 times a day.
Do not use in doses exceeding the recommended ones.

Mode of application. Remove the protective cover before use. Before the first use, press the dosing device 2-3 times to achieve the correct injection dose. Thus, the spray is ready for use.
Before using the nasal spray, you should thoroughly clean the nasal cavity. It is recommended to take the last dose of the day before bedtime.
After removing the protective cover, the hole of the dispenser must be inserted into the nasal passage and pressed once for easy breathing through the nose.
When applied, the bottle must be in an upright position. Do not use the bottle in a horizontal position.
Close the protective cover after use.
Contraindications
- Hypersensitivity to the active substance or any excipient;
- dry rhinitis (when inflammation of the nasal mucosa is not accompanied by secretion);
- glaucoma;
- transphenoidal hypophysectomy or surgery with exposure of the dura mater.

Side effects
From the immune system: very rarely (1/10 000) - hypersensitivity reactions, including angioedema, rash, itching.
From the nervous system: often (≥1 / 100-1 / 10) - headache.
From the side of the organ of vision: very rarely (1/10 000) - transient visual impairment.
From the side of the heart: very rarely (1/10 000) - rhythm disturbance or tachycardia.
On the part of the respiratory system, chest and mediastinum: often (≥1 / 100-1 / 10) - dryness or discomfort on the part of the nasal mucosa, burning sensation.
From the gastrointestinal tract: often (≥1 / 100-1 / 10) - nausea.
General disorders and reactions at the injection site: often (≥1 / 100–1 / 10) - burning sensation at the site of application.
Special instructions
Like other sympathomimetics, xylometazoline should be used with caution in patients who experience adverse reactions when using adrenergic preparations, manifested by insomnia, dizziness, tremors, arrhythmias, or increased blood pressure.
Patients who require constant treatment with xylometazoline can use this preparation for a longer period only under close clinical supervision, since there is a risk of atrophy of the nasal mucosa.
In the case of repeated use of xylometazoline, there is a risk of tachyphylaxis and iatrogenic rhinitis.
Repeated and / or prolonged instillations can cause systemic absorption of xylometazoline in amounts that cause systemic adverse reactions.
Xylometazoline should be used with caution and only if the therapeutic effect outweighs the potential risk in patients taking MAO inhibitors, antidepressants or other preparations that can increase blood pressure, patients with severe cardiovascular diseases (such as coronary artery disease, hypertension), pheochromocytoma, metabolic disorders (eg hyperthyroidism, diabetes mellitus, porphyria).
Decongestants sympathomimetics can lead to reactive hyperemia of the nasal mucosa, especially after long-term treatment or when using high doses. Repeated or chronic use of this preparation may lead to congestive withdrawal and narrowing of the upper airways. This can be manifested by chronic hyperemia of the nasal mucosa (which can further lead to atrophic rhinitis). In cases of moderate disease, the use of the preparation in one of the nasal passages can be suspended or, alternatively, not injected into both nasal passages until the intensity of symptoms decreases in order to partially preserve the patency of the nasal passages.
Multigrip Nasal contains benzalkonium chloride. It is an irritant and can cause unwanted reactions.
Multigrip Nasal Spray contains sorbitol. Patients with rare hereditary conditions of fructose intolerance should not use this preparation.
For hygiene reasons and to avoid infections, each bottle should only be used by one person.
Use during pregnancy and lactation. The preparation should not be used during pregnancy due to its potential vasoconstrictor effect. There is no evidence of any unwanted effects on infants. However, it is not known whether xylometazoline passes into breast milk, so caution must be exercised, and Multigrip Nasal should be used during breastfeeding only as directed by a doctor.
Fertility There are no adequate data on the effect of Multigrip Nasal on fertility. Since the systemic exposure of xylometazoline hydrochloride is very low, the likelihood of an effect on fertility is extremely low.

The ability to influence the reaction rate when driving vehicles or other mechanisms. Long-term treatment or the use of xylometazoline in high doses increases the risk of developing adverse reactions from the central nervous system and the cardiovascular system. If such effects occur, then it can negatively affect the ability to drive vehicles and work with other mechanisms.
Interactions
MAO inhibitors. xylometazoline can enhance the effect of MAO inhibitors and induce a hypertensive crisis. xylometazoline is not recommended for patients who have taken or have taken MAO inhibitors within the past 2 weeks.
Tri- and tetracyclic antidepressants. With the simultaneous use of tri- or tetracyclic antidepressants and sympathomimetics, it is possible to increase the sympathomimetic effect of xylometazoline, therefore, the simultaneous use of such preparations is not recommended.
Xylometazoline can interfere with the action of some antihypertensive preparations such as β-adrenergic receptor blockers.
Overdose
Excessive local use of xylometazoline hydrochloride or its accidental ingestion can lead to severe dizziness, increased sweating, a significant decrease in body temperature, headache, bradycardia, arterial hypertension, respiratory depression, coma and seizures. after an increase in hell, its decrease is possible. young children are more susceptible to toxicity than adults.
Treatment. All patients with suspected overdose should undergo appropriate supportive measures, as well as, if necessary, immediate symptomatic treatment under control. Medical care should include monitoring the patient for several hours. In the event of a severe overdose, accompanied by cardiac arrest, resuscitation should be continued for at least 1 hour.
