Pharmacological properties

Pharmacodynamics. Oxymetazoline hydrochloride is a synthetic adrenergic agonist, which, by stimulating the α-adrenergic receptors of the vessels, has a pronounced and long-term vasoconstrictor effect. the vasoconstrictor effect is manifested by a decrease in blood flow, a decrease in edema of the mucous membrane of the nasal cavity, paranasal sinuses and the Eustachian tube, as a result of which nasal breathing is restored, disturbed by influenza, colds and allergic diseases. local narrowing of the vessels of the nasal mucosa and paranasal sinuses occurs 5-10 minutes after the injection of the preparation  Nazol ® advance into the nasal cavity, the anti-edema effect lasts up to 12 hours.

Pharmacokinetics. T1 / 2 oxymetazoline hydrochloride - 5-8 hours. It is excreted mainly unchanged in the urine and feces.

Indications

Symptomatic treatment of colds, flu and ARVI, accompanied by acute rhinitis, sinusitis, other sinusitis (frontal sinusitis, ethmoiditis).

Application

Adults and children over the age of 12 years - 2 injections into each nasal passage, children aged 6–12 years - 1 injection. the bottle should be held upright. repeat the injection no earlier than 12 hours later. The duration of treatment is no more than 3 days. possibly longer use up to 7-10 days, 2 injections per day under the supervision of a physician.

Contraindications

Hypersensitivity to oxymetazoline, other adrenergic agonists or any of the components of the preparation, severe forms of ages, severe atherosclerosis, acute cardiovascular diseases or cardiac asthma, tachystolic heart rhythm disturbances, angina pectoris, renal failure, severe prostatic hypertrophy, pheochromocytoma ( hyperthyroidism, diabetes mellitus), atrophic rhinitis, increased intraocular pressure, especially with angle-closure glaucoma, after transsphenoidal hypophysectomy or other surgical intervention with the opening of the dura mater, inflammation or damage to the skin around the nasal passages or nasal mucosa, simultaneous use of preparations that cause an increase in blood pressure , simultaneous use with MAO inhibitors and use within 2 weeks after stopping treatment with MAO inhibitors.

Side effects

With frequent and prolonged use, there may be sensations of burning, tingling in the nose, flushing of the face, sneezing, dryness of the nasal mucosa. rarely - after the effect of the preparation is over, - a feeling of severe nasal congestion (reactive hyperemia). long-term continuous use of vasoconstrictor preparations can lead to tachyphylaxis or the development of rhinitis medicamentosa. generally, no severe side effects have been reported.

From the side of the cardiovascular system: tachycardia, palpitations, increased blood pressure, heart pain are very rarely possible.

From the side of the organ of vision: irritation, discomfort or redness of the eyes, blurred vision can rarely be observed.

From the gastrointestinal tract: nausea may rarely occur.

From the immune system: in rare cases, hypersensitivity reactions are possible, including rash, itching, Quincke's edema.

Respiratory, thoracic and mediastinal disorders: rarely possible discomfort or irritation in the nose, mouth and throat, dry mouth, nosebleeds.

From the nervous system: anxiety, nervousness, anxiety, insomnia, drowsiness, tremors, hallucinations (especially in children), fatigue, headache, dizziness.

On the part of the skin and subcutaneous tissues: rash.

Musculoskeletal and connective tissue disorders: convulsions (especially in children).

Special instructions

When injecting the preparation into the nasal cavity, do not throw your head back and do not turn the bottle over. use the bottle individually to avoid the spread of infection.

The persistence of swelling of the nasal passages after 3 days of using the preparation may indicate the presence of curvature of the nasal septum, purulent sinusitis, adenoids, allergic rhinitis, persistent bacterial infection or other undiagnosed diseases requiring medical advice and specialized complex therapy.

You should consult your doctor before using the preparation in the following cases: high blood pressure, heart disease, liver or kidney disorders, porphyria.

It is required to avoid prolonged use and overdose. Prolonged use of a decongestant can weaken its effect. Abuse of this preparation can cause mucosal atrophy and reactive hyperemia with rhinitis medicamentosa, as well as damage to the mucous epithelium and inhibition of epithelial activity. Doses higher than recommended can only be used under medical supervision. Do not exceed the recommended doses.

Use during pregnancy and lactation. Oxymetazoline has not been associated with adverse pregnancy outcomes. It should be used with caution in patients with hypertension or signs of decreased blood supply to the placenta. Frequent or prolonged use of high doses can lead to a decrease in placental blood flow. For women during pregnancy, the use of the preparation is possible only if, in the opinion of the doctor, the benefit from the use to the mother outweighs the potential risk to the fetus / child. It is not known whether oxymetazoline passes into breast milk. Due to the lack of data, oxymetazoline should not be used during breastfeeding.

Children. Do not use in children under 6 years of age.

The ability to influence the reaction rate when driving or operating machinery. After prolonged use of the preparation in doses exceeding the recommended ones, a general effect on the cardiovascular system is not excluded. In such cases, the ability to drive a vehicle may be impaired.

Interactions

When using other preparations at the beginning of preparation treatment, it is recommended to consult a doctor. use the preparation together with other vasoconstrictor agents (for any route of administration of the latter) or with other nasal decongestants, as well as with tricyclic antidepressants, maprotiline should be after consultation with a doctor, since blood pressure may increase.

With the simultaneous use of MAO inhibitors and oxymetazoline, an increase in blood pressure is possible. Therefore, you should not use the preparation at the same time or within 2 weeks after stopping treatment with MAO inhibitors.

With the simultaneous use of oxymetazoline with tricyclic antidepressants, the risk of increased blood pressure and the development of arrhythmias is possible.

Oxymetazoline may decrease the effectiveness of β-adrenergic blockers, methyldopa, or other antihypertensive preparations.

With the simultaneous use of sympathomimetics and antiparkinsonian preparations, an additive toxic effect on the cardiovascular system is possible.

Overdose

When applied topically, side systemic reactions usually do not occur.

Symptoms of an oxymetazoline overdose are different. The stages of hyperreactivity and inhibition are distinguished. Stimulation of the central nervous system is clinically manifested in the form of a feeling of fear, excitement, hallucinations, and seizures.

Overdose symptoms of moderate or severe: miosis (pupillary constriction), mydriasis (pupil dilation), nausea, vomiting, cyanosis, fever, increased sweating, spasms, tachycardia, arrhythmia, cardiac arrest, increased blood pressure, pulmonary edema, shortness of breath, physical discomfort.

Depression of the central nervous system is clinically manifested in the form of drowsiness, decreased body temperature, bradycardia, arterial hypotension, collapse, shock, respiratory distress and respiratory arrest, loss of consciousness, coma.

The clinical manifestations of an overdose in children include symptoms from the central nervous system: convulsions and coma, hallucinations, bradycardia, apnea, hypertension, goes into hypotension.

Urgent Care. If an overdose of oxymetazoline is suspected, urgent hospitalization is required in the intensive care unit. Activated charcoal and laxatives are used. Gastric lavage is necessary. In order to reduce elevated blood pressure, α-adrenergic receptor blockers (phentolamine) should be prescribed. It is not recommended to take vasopressors. Anticonvulsant therapy is indicated if necessary. In severe cases, intubation and mechanical ventilation may be necessary.
