Product description
Tablets with modified release "Olfen®-AF" are used for the following indications: pain syndrome in rheumatoid arthritis, ankylosing spondylitis, osteoarthritis and humeral periarthritis, lumbago, sciatica, extra-articular rheumatism.
Compound
The active ingredient is aceclofenac (one tablet contains 200 mg aceclofenac).
Excipients: microcrystalline cellulose, povidone, croscarmellose sodium, sodium stearyl fumarate, poloxamer, hypromellose, carbomer, Opadry White (OY-C-7000A).
Contraindications
- hypersensitivity to the active substance or to the auxiliary substances of the preparation;
- a history of bleeding or perforation in the gastrointestinal tract associated with previous treatment with non-steroidal anti-inflammatory preparations (NSAIDs);
- like other NSAIDs, aceclofenac is also contraindicated in patients in whom the use of acetylsalicylic acid or other NSAIDs provokes attacks of bronchial asthma, angioedema, urticaria or acute rhinitis;
- bronchial asthma;
- history of active peptic ulcer / bleeding or recurrent peptic ulcer / bleeding (two or more separate episodes of established ulcer or bleeding);
- active bleeding or bleeding disorders;
- severe hepatic or renal impairment;
- congestive heart failure (NYHA II-IV);
- cardiac ischemia;
- peripheral arterial disease;
- cerebrovascular diseases, including stroke or episodes of transient ischemic attack;
- treatment of perioperative pain with coronary artery bypass grafting (or the use of a heart-lung machine);
- inflammatory bowel disease (such as Crohn's disease or ulcerative colitis);
- III trimester of pregnancy or lactation period;
- childhood.
Mode of application
Olfen®-AF tablets should be swallowed whole without chewing or crushing. Drink at least ½ glass of water.
Adverse events can be minimized by using the preparation for the minimum period necessary to control symptoms.
Adults
The recommended dose is 200 mg once a day (every 24 hours).
Elderly patients
Dose reduction is generally not required. However, the condition of such patients should be carefully monitored, because they are more often diagnosed with impaired renal function, liver, cardiovascular disorders, and they also more often receive concomitant therapy for other diseases, which increases the risk of developing serious consequences of adverse reactions. If it is necessary to prescribe NSAIDs, they should be used in minimal doses and for the shortest possible time. Patients should be closely monitored for timely detection of gastrointestinal bleeding during NSAID therapy.
Liver failure
For patients with mild to moderate hepatic impairment, the dose of aceclofenac should be reduced. The recommended starting dose is 100 mg per day (use aceclofenac in a different dosage form).
Renal failure
There is no information that dose adjustment of aceclofenac is necessary in patients with mild renal insufficiency, however, this category of patients should be careful when using the preparation.
Application features
Pregnant
There is no information on the use of aceclofenac during pregnancy. Inhibition of prostaglandin synthesis can have a negative impact on pregnancy / fetal development. Based on the known effects of NSAIDs on the fetal cardiovascular system (risk of premature closure of the ductus arteriosus) and the possible risk of persistent pulmonary hypertension in newborns, the use of the preparation in the last trimester of pregnancy is contraindicated. The risk increases with dose and duration of therapy. Regular use of NSAIDs in the last trimester of pregnancy can reduce the tone and contractility of the uterus. As a result, labor can be delayed or protracted, with an increased tendency for mother and baby to bleed. NSAIDs should not be used in the first two trimesters of pregnancy and childbirth, unless the potential benefit to the patient outweighs the potential risk to the fetus, the dose should be as low as possible, and the duration of treatment should be as short as possible.
Limited data available indicate that NSAIDs are found in breast milk at very low concentrations. Taking these preparations during breastfeeding should be avoided if possible. If treatment is necessary, breastfeeding should be discontinued.
NSAIDs can interfere with fertility and are therefore not recommended for women trying to get pregnant. Women who are trying to get pregnant or are being tested for infertility should temporarily stop using aceclofenac.
Drivers
After taking NSAIDs, side effects such as dizziness, drowsiness, fatigue, and visual disturbances may occur. In the event of such reactions, patients should not drive a car or operate machinery.
Overdose
Symptoms
Symptoms include headache, nausea, vomiting, epigastric pain, irritation and bleeding from the gastrointestinal tract, rarely diarrhea, disorientation, agitation, coma, drowsiness, dizziness, tinnitus, hypotension, respiratory distress, loss of consciousness, sometimes convulsions. In the case of severe poisoning, acute renal failure and liver damage can develop.
Treatment methods
Patients should receive symptomatic treatment as needed. Activated charcoal should be taken within one hour after ingestion of potentially toxic amounts of the preparation. Alternatively, adults should undergo gastric lavage within one hour of a life-threatening overdose. Specific therapeutic agents such as dialysis or hemoperfusion are likely to be ineffective in removing NSAIDs due to their high protein binding and extensive metabolism. It is necessary to provide forced diuresis. Close monitoring of kidney and liver function is needed. Patients should be monitored for at least 4 hours after swallowing potentially toxic amounts of the preparation. In the case of frequent and prolonged seizures, the patient needs intravenous diazepam. Treatment is symptomatic and supportive.
Side effects
The most common side effects of aceclofenac include gastrointestinal upset, dizziness, and increased liver enzyme activity.
Storage conditions
Store in its original packaging at a temperature not exceeding 30 ° C, out of the reach of children.
Shelf life is 2 years.
