Pharmacological properties

Secrepat forte is a combined balanced antacid preparation for the elimination of heartburn and stomach pain. a decrease in the severity of pain and heartburn is provided by the effects of the components that make up the preparation.

Calcium carbonate acts within a few minutes after ingestion, neutralizing hydrochloric acid.

Due to the floating effect, magnesium trisilicate promotes the formation of an antacid film on the surface of the gastric contents, which covers the wall of the esophagus during reflux. Aluminum salts (dihydroxyaluminium aminoacetate and aluminum hydroxide) in the composition of Secrepat Forte have acid-neutralizing, enveloping, adsorbing effects, maintaining a pH of about 4. Due to its buffering properties, only excess hydrochloric acid is neutralized, preserves the physiological conditions of digestion and does not lead to secondary hypersecretion of gastric juice. The cytoprotective effect of aluminum salts is due to the ability to activate their own protective factors, in particular the synthesis of prostaglandins, which stimulate the secretion of mucus and bicarbonates in the gastric mucosa, which leads to the accumulation of epidermal growth factor in the defect area and ulcers scarring.

In the process of neutralization, carbon dioxide is not formed, there is no flatulence and, as a result, no secondary hypersecretion. The preparation acts locally. Only a small amount of aluminum compounds is absorbed, slightly more - magnesium compounds, which are quickly excreted from the body in the urine. Due to the neutralization of hydrochloric acid, aluminum and magnesium hydroxides are converted to chlorides and excreted in the feces.

Indications

Symptomatic treatment for pain and heartburn in diseases of the digestive tract: peptic ulcer of the stomach and duodenum, gastroesophageal reflux disease, dyspepsia, acute and chronic gastritis with increased or normal acid-forming stomach function. irritation of the gastric mucosa caused by smoking or errors in the diet (alcohol, coffee, sweets, flour products, etc.). with a prophylactic purpose - with prolonged use of NSAIDs, corticosteroids and other ulcerogenic preparations.

Application

The preparation Secrepat Forte is taken orally. the tablets dissolve or chew slowly. adults are prescribed 1-2 tablets 4-6 times a day, the last time - just before bedtime. the preparation  is recommended to be taken 1–1.5 hours after meals. the course of treatment is 3-4 weeks. for heartburn or pain syndrome, the preparation is taken in the same dose, regardless of food intake (on demand). To protect the gastric mucosa from the irritating effect of ulcerogenic preparations, take 1-2 tablets of Secrepat Forte 2 hours before taking them. children aged 10-18 years are prescribed ½ the dose for adults. the maximum daily dose for adults is 12 tablets.

Contraindications

Hypersensitivity to the components of the preparation, severe renal failure (creatinine clearance 30 ml / min); nephrolithiasis due to the formation of calcium-containing stones; Alzheimer's disease; hypophosphatemia; constipation, chronic diarrhea, severe abdominal pain of unspecified origin, hypercalcemia and / or conditions leading to hypercalcemia; the patient's age is up to 10 years.

Side effects

The preparation is usually well tolerated by patients. Secrepat forte balances the possible astringent properties of aluminum and calcium with the laxative properties of magnesium. however, side reactions may occur.

From the immune system: hypersensitivity reactions, including rash, pruritus, urticaria, angioedema and anaphylaxis.

Metabolic disorders. In patients, especially those with impaired renal function, long-term use in high doses can lead to hypermagnesemia (when using antacids containing magnesium) or hypercalcemia and alkalosis. Hypercalciuria, hyperaluminemia may also be note

PROPER USE SAFETY

The recommended dose of the preparation should not be exceeded. The preparation should not be taken at the same time with large amounts of milk or dairy products.

Aluminum salts are generally poorly absorbed in the gastrointestinal tract, so systemic effects are rare in patients with normal renal function. However, the use of the preparation in high doses or long-term use of the preparation, or even the use of the preparation in normal doses in patients whose diet is characterized by a low phosphorus content, can lead to a decrease in the phosphate content in the body, which is accompanied by an increase in bone resorption processes and the occurrence of hypercalciuria with an increased risk of osteomalacia (due to the binding of aluminum to phosphate).

In patients with renal insufficiency, elevated plasma concentrations of both aluminum and magnesium may be observed. In such patients, long-term use of aluminum and magnesium salts in high doses can lead to the development of encephalopathy, dementia, microcytic anemia, or worsen the course of dialysis-induced osteomalacia.

Aluminum hydroxide can be hazardous to porphyria patients on hemodialysis.

Calcium and magnesium salts should not be used for hypercalciuria. With caution, Sekrepat forte is prescribed to patients with impaired renal function, especially those on dialysis, with metabolic disorders and patients on a low-phosphate diet to reduce the risk of intoxication with aluminum, magnesium and the development of hypophosphatemia. With prolonged use of the preparation, it is necessary to control the level of aluminum, magnesium and calcium in the blood plasma. Long-term use of the preparation in high doses may increase the risk of kidney stones.

Patients with hereditary diseases accompanied by fructose intolerance, glucose-galactose malabsorption or sucrose-isomaltose deficiency should not take the preparation because of the sucrose content.

Patients with diabetes mellitus should take into account that the composition of the preparation includes sucrose.

During pregnancy and breastfeeding. During pregnancy, the use of the preparation is possible as directed and under the supervision of a physician if the expected benefit to the mother outweighs the potential risk to the fetus. It is necessary to take into account the content of aluminum and magnesium ions that can affect transit in the gastrointestinal tract:

• magnesium salts can cause diarrhea;
• aluminum salts can cause constipation and worsen the constipation often seen during pregnancy.

The presence of calcium in high doses, used for a long time, increases the risk of hypercalcemia with calcification of various organs, in particular the kidneys.

Long-term use and exceeding doses of this preparation should be avoided.

When using the preparation in pregnant women, constant consumption of milk (1 liter contains up to 1.2 g of elemental calcium) should be avoided, as well as dairy products to prevent excess calcium, which can lead to the so-called lactic-alkaline syndrome (Burnett's syndrome), which is rare , but a serious illness, and requires medical attention.

If it is necessary to prescribe the preparation, breastfeeding should be discontinued for the entire period of its use.

Interactions

The 2-hour interval between taking the preparation Secrepat Forte and the following preparations  should be observed: ethambutol, chloroquine, cyclins, diflunisal, diphosphonates, fexofenadine, sodium fluoride, corticosteroids (interaction with prednisolone and dexamethasone is described), indomethacin, cayexalate, ketoconazole, lansopami , phenothiazine antipsychotics, penicillamine, phosphorus (additives), thyroxine.

Secrepat forte reduces the absorption of certain preparations (tetracyclines, fluoroquinolones, isoniazid, naproxen, quinidine, cardiac glycosides (digoxin), histamine receptor blockers, adrenergic receptor blockers, iron preparations).

The preparation increases the absorption of levodopa, acetylsalicylic and nalidixic acid.

Thiazide diuretics reduce urinary calcium excretion and increase plasma levels. Due to the increased risk of hypercalcemia with the simultaneous use of thiazide diuretics, it is necessary to regularly monitor the level of calcium in the blood plasma.

In the case of simultaneous use with phosphates, fluorine compounds, levothyroxine and eltrombopag, the absorption of the latter decreases.

The simultaneous use of aluminum hydroxide and citrates can lead to an increase in the level of aluminum, especially in patients with renal insufficiency.

Overdose

The preparation in high doses, as well as with prolonged use (more than 4 weeks) can cause nausea, vomiting, constipation, diarrhea, loss of appetite. renal failure may also occur, especially in patients with impaired renal function, hypercalcemia, alkalosis, electrolyte disturbances (a decrease in the concentration of phosphates and an increase in the concentration of magnesium and aluminum in the blood plasma), which are most likely with chronic renal failure, muscle weakness. cases of acute overdose and symptoms of acute toxicity in humans were not observed. if the above symptoms occur, it is necessary to stop taking the preparation and rinse the stomach.
