Pharmacological properties

Pharmacodynamics. The active ingredient of the preparation fluocinolone acetonide is a fluorinated glucocorticoid.

When applied topically, it has anti-inflammatory, anti-allergic, antipruritic effect. Binds to specific receptors located in the cytoplasm of target organ cells and activates mRNA synthesis; induces the formation of lipocortin proteins, one of which, ipomodulin, inhibits the activity of phospholipase A2. Affects all types of metabolism. Accelerates protein catabolism. Reduces the utilization of glucose by insulin-dependent tissues, causes hyperglycemia, increases the amount of glycogen, enhances the breakdown of fats, retains sodium and water ions, promotes the excretion of potassium, reduces the absorption of calcium ions, causes hypocalcemia, induces the secretion of parathyroid hormone, which mobilizes calcium from bone tissue. Reduces the number of lymphocytes and eosinophils, stimulates the production of erythropoietins. The anti-inflammatory effect is due to the inhibition of the activity of phospholipase A2, as well as the stabilization of cell membranes and organelles, in particular lysosomes. Inhibits the formation of arachidonic acid and its metabolic products: prostaglandins, thromboxane, leukotrienes. Inhibits hyaluronidase; suppresses the division of mast cells, prevents their degranulation, reducing the release of inflammatory mediators: histamine, serotonin, bradykinin. Suppresses the synthesis of platelet activating factor. Reduces the proliferation of connective tissue. Immunosuppressive and antiallergic effects are a consequence of a decrease in the migration and number of B- and T-lymphocytes, a violation of their interaction.

Pharmacokinetics. Fluocinolone acetonide easily penetrates the skin through its outer stratum corneum. It is not metabolized in the skin, but accumulates in its stratum corneum; it can be detected even 15 days after application. After absorption through the skin, the preparation is metabolized mainly in the liver, excreted in the urine and, in small amounts, in the bile, in the form of glucuronide conjugates - unchanged.

Indications

Short-term treatment of acute and severe non-infectious inflammatory skin diseases (dry forms), which are accompanied by persistent itching or hyperkeratosis: seborrheic dermatitis, atopic dermatitis, urticaria, contact allergic eczema, polymorphic erythema, tuberculous lupus, psoriasis, lichen planus.

Application

The preparation is intended for topical use. the ointment should be applied to the skin in a thin layer 1-2 times a day. do not apply ointment under an occlusive dressing. only in psoriasis is it allowed to use a closed dressing, which must be changed daily. treatment should not be carried out continuously for longer than 2 weeks. can not be applied on the skin of the face for more than 1 week. during the week it is recommended to use no more than 1 tube (15 g of ointment). the preparation should be used with caution and under the supervision of a physician in children aged 2 years, only once a day, on a small area of ​​the skin; do not apply to the skin of the face. the duration of the course of treatment is usually 1–2 weeks.

Contraindications

Hypersensitivity to the components of the preparation and other corticosteroids. infectious skin diseases (bacterial, viral, fungal), including skin manifestations of syphilis, skin tuberculosis, pyoderma, rosacea and acne, diaper dermatitis, anogenital itching, perioral dermatitis, skin tumors, trophic ulcers associated with varicose veins. glaucoma, cataracts. During pregnancy and breastfeeding. age up to 2 years. should not be used after prophylactic vaccinations.

Side effects

Allergic reactions: in some cases, urticaria, allergic contact dermatitis may appear;

on the part of the skin and its appendages: burning sensation, skin itching, skin irritation, atrophic streaks, maculopapular rash, post-steroid purpura, maceration or dryness of the skin at the application site, excessive body hair growth or alopecia, discoloration or hyperpigmentation of the skin, atrophy and violation of the integrity of the skin, hyperkeratosis, striae, telangiectasia, perioral dermatitis, furunculosis, acne-like rashes, delayed healing of wounds and ulcerative lesions, the development of secondary infectious skin lesions and exacerbation of existing skin diseases are possible;

from the gastrointestinal tract: when applied to large areas of the skin, the development or exacerbation of chronic gastritis, steroid gastric ulcer is possible;

from the endocrine system: with prolonged use on large areas of the skin, systemic manifestations are possible (insufficiency of the adrenal glands, Itsenko-Cushing's syndrome, hyperglycemia);

on the part of the cardiovascular system: hypertension;

from the urinary system: the development of edema is possible;

from the senses: when applied externally, cataracts or glaucoma can sometimes develop on the skin of the eyelids.

Special instructions

Do not use continuously for more than 2 weeks. with prolonged use on large areas of the skin, the frequency of side effects and the likelihood of developing edema, ages, hyperglycemia, and a decrease in resistance increase.

If an infection develops at the site of application of the ointment, appropriate antibacterial or antifungal treatment must be carried out.

When using the preparation, it is also possible to reduce the production of adrenocorticotropic hormone by the pituitary gland due to inhibition of the renal glands-pituitary gland system, a decrease in the level of cortisol in the blood and the development of iatrogenic Cushing's syndrome, which disappears after the preparation is discontinued. Avoid applying the preparation to the eyelids or skin around the eyes in patients with closed-angle and open-angle glaucoma, as well as in patients with cataracts, given the possibility of intensifying the symptoms of the disease.

On the skin of the face, in the armpit, and also in the groin area, apply only if there are direct indications, since the absorption of the preparation may increase, and the risk of side effects (telangiectasia, perioral dermatitis) also increases, even after short-term use.

Under special control, the ointment should be used in patients with psoriasis, since the local use of glucocorticoids in areas affected by psoriasis can lead to the spread of the pathological process or relapse caused by the development of tolerance, as well as the risk of generalized pustular psoriasis and systemic toxicity caused by skin dysfunction.

The preparation should be used with caution in the presence of atrophy of the subcutaneous tissue, mainly in the elderly. During treatment, it is not recommended to vaccinate against smallpox, as well as to carry out other types of immunization.

Do not apply the ointment at the same time as other medicines for external use.

Use during pregnancy and lactation. The preparation is contraindicated during pregnancy. During treatment with the preparation, breastfeeding should be discontinued.

Children. The preparation is not used in children under 2 years of age.

The ability to influence the reaction rate when driving or working with other mechanisms. The preparation in the recommended doses does not affect the reaction rate when driving or operating other mechanisms.

Interactions

Systemic corticosteroids preparations increase the effectiveness of the preparation, but at the same time increase the likelihood of side effects. combined use with NSAIDs increases the risk of systemic and local side effects. reduces the activity of antihypertensive, diuretic, antiarrhythmic preparations, potassium preparations. diuretic preparations  (other than potassium-sparing preparations) increase the likelihood of hypokalemia.

During treatment with the preparation, it is not recommended to vaccinate against smallpox, as well as to carry out other types of immunization (especially with prolonged use on large areas of the skin) due to the possible lack of an adequate immunological response in the form of the production of appropriate antibodies.

Sinaflan-Fitofarm can enhance the effect of immunosuppressive preparations and reduce the effectiveness of immunostimulating preparations.

Overdose

Itching, burning at the site of application of the ointment, edema, ag, a decrease in the body's resistance to infection, hyperglycemia, glucosuria, itsenko-cushing's syndrome. symptomatic treatment against the background of gradual withdrawal of the preparation.
