The maximum single dose is calculated in mg of lidocaine hydrochloride per 1 kg of body weight (2.9 mg of lidocaine hydrochloride / kg of body weight).

Hypersensitivity to one of the components of the preparation, as well as hypersensitivity to other local anesthetics of the amide type; bulbocavernous (urethrocavernous) reflux; severe heart failure (IV functional class according to the nyha classification); severe bradycardia (33 beats / min); severe av-blockade (III degree); cardiogenic shock; hypovolemic shock.

Adverse reactions after using the preparation  cathejel with lidocaine appear rarely (1/10 000) if the preparation is used according to the dosage recommendations and the route of administration.

Rarely, there may be cases of local and / or systemic hypersensitivity reactions to lidocaine and / or chlorhexidine.

Avoid getting the preparation cathejel with lidocaine in the eyes. the preparation should be used with caution in patients with severe hepatic dysfunction (stage iii and iv), renal dysfunction (creatinine clearance 30 ml / min), in severe cardiac or respiratory dysfunction (stage iii-iv).

Use during pregnancy and lactation. During pregnancy, Cathejel with lidocaine can be used only after a thorough analysis by the doctor of the benefit / risk ratio and after the individual dose has been established. Re-use of the preparation during pregnancy is not recommended. During lactation, Katejel with lidocaine should be used only when necessary; doses should be selected individually. The interval between the instillation of Cathejel with lidocaine and the next breastfeeding should be 12 hours. Repeated use of the preparation during breastfeeding is not recommended.

Children. You should not appoint Catejel with lidocaine to children under the age of 2 years.

The ability to influence the reaction rate when driving or operating other mechanisms. Cathejel with lidocaine practically does not affect the ability to drive vehicles or work with other mechanisms, but its effect cannot be completely excluded in cases of increased individual sensitivity.

Cathejel with lidocaine should not be used in combination with preparations containing lidocaine hydrochloride, or with other local anesthetics of the amide type, as this can lead to an unpredictable mutual influence of preparations.

Due to the possible additive effect on the heart muscle, lidocaine should be used with caution in patients who are simultaneously receiving antiarrhythmic preparations such as mexiletine and tocainide, β-adrenergic receptor blockers (eg propranolol) or calcium channel antagonists (eg diltiazem, verapamil).

Propranolol, diltiazem and verapamil lead to a significant increase in T½ due to a decrease in the clearance of lidocaine. Preparations that decrease the clearance of lidocaine can lead to potentially toxic plasma concentrations if lidocaine is reused in high doses for a long time. Therefore, with short-term use of Katejel with lidocaine in recommended doses, such interactions have no clinical significance.

Special studies of the interaction of lidocaine and class III antiarrhythmics (for example, amiodarone) have not been conducted, however, it is recommended to be careful when using them in combination.

Care must be taken when using Catejel with lidocaine and cimetidine, an H2-receptor antagonist, in combination. The concentration of lidocaine in blood plasma may increase due to a decrease in hepatic perfusion and inhibition of microsomal enzymes.

With the correct use of cathejel with lidocaine in the blood plasma, the toxic concentration of the preparation (5 μg / ml) is not achieved. however, combined use with other local anesthetics can cause an additive effect, which, in turn, can lead to preparation  overdose with the subsequent development of systemic toxic reactions.
