Product description

Diroton tablets are used for the following indications:
  • arterial hypertension;
  • heart failure (symptomatic treatment);
  • acute myocardial infarction (short-term treatment for 6 weeks in hemodynamically stable patients no later than 24 hours after acute myocardial infarction);
  • treatment of kidney diseases in patients with arterial hypertension, patients with type II diabetes mellitus and initial nephropathy.

Compound

Active substance: lisinopril;
1 tablet contains 5 mg or 10 mg or 20 mg of lisinopril in the form of lisinopril dihydrate;
Excipients: magnesium stearate talc lures (E 421) corn starch calcium hydrogen phosphate dihydrate.

Contraindications

  • hypersensitivity to the active or auxiliary substance of the preparation;
  • hypersensitivity to any other ACE inhibitor;
  • a history of angioedema associated with previous treatment with other ACE inhibitors;
  • hereditary or idiopathic angioedema;
  • pregnant women or women planning to become pregnant;
  • lactation period;
  • simultaneous use of the preparation Diroton with preparations containing aliskiren in patients with diabetes mellitus or renal failure (glomerular filtration rate <60 ml / minute / 1.73 m 2).

Mode of application

The dosage should be individualized according to the patient's illness and blood pressure response. It is taken orally once a day, every day at the same time, regardless of the meal.

Application features

Pregnant
The preparation is contraindicated for pregnant women or women planning pregnancy. If pregnancy is confirmed during treatment with this preparation, its use must be stopped immediately and, if necessary, replaced with another preparation approved for use in pregnant women.
Since there is no information on the possibility of using lisinopril during breastfeeding, taking lisinopril during this period is contraindicated.
Children
In hypertensive children> 6 years of age, the preparation has limited efficacy and safety, but no other indication is available. Lisinopril is not recommended for the treatment of children for indications other than arterial hypertension. Lisinopril is not recommended for the treatment of children under 6 years of age or children with severe renal impairment (GFR <30 ml / minute / 1.73m 2).
Drivers
Given the possibility of dizziness and the development of fatigue, lisinopril may affect the ability to drive vehicles and other mechanisms, especially at the beginning of treatment. Therefore, one should refrain from driving vehicles and working with other mechanisms until an individual reaction to the preparation is established.

Overdose

Data on overdose of the preparation Diroton in humans are limited. Symptoms associated with an overdose of ACE inhibitors may include hypotension, circulatory shock, electrolyte imbalance, renal failure, hyperventilation, tachycardia, heart palpitations, bradycardia, dizziness, anxiety, and cough.
In case of overdose, the introduction of saline is recommended. In cases of arterial hypotension, the patient must be placed in a horizontal position. If possible, infusion of angiotensin II and / or intravenous administration of catecholamines can be used. If the preparation has been used recently, measures should be taken to remove lisinopril from the body (for example, induce vomiting, rinse the stomach, take absorbents and sodium sulfate). Lisinopril can be removed from the general circulation by hemodialysis. It is necessary to frequently check vital organs, the concentration of electrolytes and serum creatinine. The use of a pacemaker is indicated for therapy-resistant bradycardia.

Side effects

From the nervous system: frequent (from ≥ 1/100 to <1/10) - dizziness, headache.
Vascular disorders: frequent (≥ 1/100 to <1/10) - orthostatic disorders (including hypotension).
From the respiratory system, chest and mediastinal organs: frequent (from ≥ 1/100 to <1/10) - cough.
From the digestive tract: frequent (from ≥ 1/100 to <1/10) - diarrhea, vomiting.
On the part of the kidneys and urinary system: frequent (from ≥ 1/100 to <1/10) - impaired renal function.

Interaction

Concomitant use of antidiabetic agents (insulin, oral hypoglycemic preparations) with ACE inhibitors can lead to an increase in the effect of lowering blood glucose with the risk of hypoglycemia. This phenomenon is usually more likely to manifest itself in the first week of combination treatment and in patients with renal insufficiency.
