Pharmacodynamics. The properties of lorinden s ointment are due to the combined action of flumethasone pivalate and clioquinol. flumethasone pivalate is a synthetic corticosteroids with a moderate anti-inflammatory effect. in the case of application in the form of an ointment, it has a local effect of medium intensity. possesses lipophilic properties, easily penetrates into the stratum corneum and exhibits long-term anti-inflammatory, antipruritic and anti-allergic effects. when applied externally, flumethasone pivalate inhibits the formation of prostaglandins and leukotrienes in the skin due to inhibition of the activity of phospholipase a2 and a decrease in the release of arachidonic acid from phospholipids of cell membranes. suppresses phagocytosis, the release of interleukins and other cytokines, the release of histamine and the manifestation of local allergic reactions, reduces proliferative processes. due to the local vasoconstrictor action, it reduces the severity of the exudative reaction. reduces protein synthesis and collagen deposition.

Clioquinol has an antibacterial effect against gram-positive and gram-negative bacteria, as well as a weak antifungal effect.

Pharmacokinetics. Flumethasone pivalate easily penetrates into the stratum corneum, where it accumulates. It is not metabolized in the skin. When applied externally in a small amount, it can enter the body and in this case has a systemic effect. After penetration through the skin, the preparation is metabolized mainly in the liver, excreted in the urine and in small quantities with bile in the form of glucuronide conjugates, as well as unchanged. The absorption of flumethasone pivalate is increased when applied to areas with delicate skin - in the area of ​​folds or the face, on skin with damaged epidermis or inflammatory skin changes. The use of an occlusive dressing, which increases the temperature and humidity of the skin, also increases the absorption of flumethasone pivalate. In addition, absorption is increased with frequent use of the preparation or when applied to large areas of the skin. Absorption through the skin in young people is more intense than in older people.

Clioquinol has a local effect and is practically not absorbed into the bloodstream.

Local treatment of skin lesions without exudation, complicated by a bacterial infection, which are accompanied by itching and hyperkeratosis: seborrheic dermatitis, atopic dermatitis, allergic contact dermatitis, erythema multiforme, systemic lupus erythematosus, psoriasis, lichen planus.

The preparation is intended for topical use. the ointment is applied in a thin layer to the affected area of ​​the skin 1-2 times a day. treatment should not be carried out continuously for more than 2 weeks. can not be applied on the face for more than 1 week. within a week it is recommended to use no more than 1 tube.

With increased lichenization or excessive hyperkeratosis, it is allowed to use an occlusive dressing, which should be changed every 24 hours.

Hypersensitivity to flumethasone pivalate, other corticosteroids, clioquinol or other components of the ointment. viral (eg chickenpox, herpes simplex), fungal or bacterial (eg tuberculosis) infections of the skin, skin neoplasms, acne and rosacea, perioral and diaper dermatitis, anal itching, itching in the vulva, inflammation or ulcers of varicose veins.

Do not use on large areas of the skin, especially if the integrity of the skin is compromised, for example, in case of burns.

On the part of the skin and subcutaneous tissue: acne, post-steroid purpura, inhibition of epidermal growth, atrophy of the subcutaneous tissue, dry skin, burning sensation, irritation, itching, excessive body hair growth or alopecia, skin discoloration or hyperpigmentation, atrophy and skin integrity disorders, telangiectasia , perioral dermatitis, inflammation of the hair follicles, secondary infections. in some cases, there may appear: urticaria, maculopapular rash or exacerbation of existing changes.
