Pharmachologic Effect

Antiulcer preparation. Specific proton pump inhibitor: inhibits the activity of H + -K + -ATPase in parietal cells of the stomach, blocking the final stage of hydrochloric acid secretion, thereby reducing the production of hydrochloric acid.

Omeprazole is a prodrug and is activated in the acidic environment of the secretory tubules of the parietal cells of the stomach.

The effect is dose-dependent and provides effective inhibition of both basal and stimulated acid secretion, regardless of the nature of the stimulating factor.

The antisecretory effect after taking 20 mg occurs within the first hour, maximum - after 2 hours. Inhibition of 50% of the maximum secretion lasts 24 hours.

A single dose per day provides a quick and effective suppression of daytime and nocturnal gastric secretion, reaching its maximum after 4 days of treatment and disappearing by the end of 3-4 days after the end of the intake. In patients with duodenal ulcer, taking 20 mg of omeprazole maintains an intragastric pH above 3 for 17 hours.

Indication for use

• peptic ulcer of the stomach and duodenum (including prevention of relapse);
• reflux esophagitis;
• hypersecretory conditions (Zollinger-Ellison syndrome, stress gastrointestinal ulcers, polyendocrine adenomatosis, systemic mastocytosis);
• NSAID gastropathy;
• eradication of Helicobacter pylori in infected patients with gastric ulcer and duodenal ulcer (as part of combination therapy).

Methods of administration and dosage

It is taken orally with a small amount of water (the contents of the capsule cannot be chewed), 30 minutes before meals.

With exacerbation of gastric ulcer and duodenal ulcer, reflux esophagitis and NSAID gastropathy - 20 mg 1 time / day. For patients with severe reflux esophagitis, the dose is increased to 40 mg 1 time / day. The course of treatment for duodenal ulcer is 2-3 weeks, if necessary - 4-5 weeks; ulcerative stomach disease and reflux esophagitis - 4-8 weeks.

With resistance to treatment with other antiulcer preparations, Omez® is prescribed at a dose of 40 mg / day. The course of treatment for duodenal ulcer is 4 weeks, for gastric ulcer and reflux esophagitis - 8 weeks.

In Zollinger-Ellison syndrome, the recommended dose is 60 mg / day; if necessary, the dose is increased to 80-120 mg / day (in this case, it is prescribed in 2-3 doses).

For the prevention of Mendelssohn's syndrome - 40 mg 1 hour before the operation (if the duration of the operation is more than 2 hours, the required dose of the preparation should be re-administered).

For the prevention of recurrence of gastric ulcer and duodenal ulcer - 20 mg 1 time / day.

For the eradication of Helicobacter pylori, "triple" therapy is used (for 1 week: omeprazole 20 mg, amoxicillin 1 g, clarithromycin 500 mg - 2 times / day; or omeprazole 20 mg, clarithromycin 250 mg, metronidazole 400 mg - 2 times / day; or omeprazole 40 mg - 1 time / day, amoxicillin 500 mg and metronidazole 400 mg - 3 times / day) or "double" therapy (within 2 weeks: omeprazole 20-40 mg and amoxicillin 750 mg - 2 times / day; or omeprazole 40 mg - 1 time / day and clarithromycin 500 mg - 3 times / day or amoxicillin 0.75-1.5 g - 2 times / day).

Special instructions

Before starting therapy, it is necessary to exclude the presence of a malignant process (especially with gastric ulcer), because Treatment by masking symptoms may delay the correct diagnosis.

Taking the preparation simultaneously with food does not affect its effectiveness.

If you have difficulty swallowing a whole capsule, the contents of the capsule, after carefully opening it, can be added to the applesauce.
