Pharmacological properties

Pharmacodynamics. An enzyme preparation from the pancreas of cattle and pigs. pancreatic enzymes that make up the preparation- lipase, amylase and protease facilitate the process of digestion of proteins, carbohydrates and food fats, contributing to a more complete absorption of nutrients in the small intestine. in diseases of the pancreas, the preparation compensates for the insufficiency of its exocrine function and helps to improve the digestion process.

Pharmacokinetics. Due to the acid-resistant coating, enzymes are not inactivated by the action of gastric hydrochloric acid. The dissolution of the membrane and the release of enzymes begins in the duodenum. Enzymes are poorly absorbed in the digestive tract and act in the intestinal lumen.

Indications

Diseases accompanied by impaired digestion of food due to insufficient secretion of digestive enzymes by the pancreas, such as chronic pancreatitis. conditions after simultaneous resection of the stomach and small intestine; functional acceleration of the passage of food through the intestines, intestinal disorders, the simultaneous use of indigestible vegetable, fatty and unusual food. bloating of the intestines. preparation for X-ray or ultrasound diagnostic examinations.

Application

The dose of the preparation pancreatin forte depends on the deficiency of pancreatic enzymes in the duodenum and is set by the doctor individually.

If there are no other recommendations, as well as when using indigestible plant foods, fatty or unusual foods, take 1-2 tablets. In other cases, when digestive disorders occur, the recommended dose is 2–4 tablets. If necessary, the dose of the preparation can be increased. Increasing the dose in order to reduce symptoms of the disease, such as steatorrhea or abdominal pain, should only be carried out under the supervision of a physician. The daily dose of lipase should not exceed 15,000–20,000 lipolytic U Ph. Eur. per 1 kg of body weight.

The tablets are swallowed whole without chewing, drinking plenty of liquid, for example 1 glass of water.

Contraindications

Side effects

The preparation contains active enzymes that can damage the mucous membrane of the oral cavity, so the tablets must be swallowed whole without chewing.

In order to avoid the formation of uric acid calculi with intestinal obstruction, the content of uric acid in the urine should be monitored.

The preparation reduces the absorption of iron, therefore, with prolonged use, iron supplements should be prescribed at the same time. Gastrointestinal disorders may occur in patients with hypersensitivity to pancreatin or in patients with a history of bowel resection.

The use of preparations containing pancreatin can reduce the absorption of folic acid, which may necessitate its additional intake into the body.

The medicinal product contains lactose, therefore, if the patient has an intolerance to some sugars, you should consult your doctor before taking this medication.

The medicinal product contains sodium croscarmellose.

Use during pregnancy and lactation. During pregnancy and lactation, the preparation should be taken as directed by a doctor if the expected benefit to the mother outweighs the potential risk to the fetus / child.

Interactions

With the simultaneous use of pancreatin with antithrombotic agents, vitamin K antagonists and acetylsalicylic acid, the effect of these preparations is reduced. also decreases the effectiveness of non-selective inhibitors of neuronal reuptake of monoamines when combined with pancreatin.

With simultaneous use with M-anticholinergics, the anticholinergic effect is enhanced.

The simultaneous use of the preparation Pancreatin forte with iron and folic acid preparations can lead to a decrease in their absorption, a decrease in the hypoglycemic effect of acarbose.

When using pancreatin, it is possible to reduce the absorption of iron and folic acid. The simultaneous use of antacids containing calcium carbonate and / or magnesium hydroxide, with tannin, alcohol-containing agents can lead to a decrease in the effectiveness of pancreatin.

Overdose

Symptoms when very high doses of pancreatin were taken, hyperuricemia and hyperuricosuria, an increase in the level of uric acid in the blood plasma were observed.
