Pharmacological properties

Pharmacodynamics. The active substance of the preparation Phencarol is a derivative of quinuclidylcarbinol, which reduces the effect of histamine on organs and systems. Chifenadine is a competitive H1 receptor blocker. in addition, it activates the enzyme diamine oxidase, which breaks down about 30% of endogenous histamine. this explains the effectiveness of hifenadine in patients who are insensitive to other antihistamines. hifenadine poorly penetrates the geb and has little effect on the processes of deamination of serotonin in the brain, has little effect on the activity of mao. the antihistaminic properties of hifenadine are associated with the presence of a cyclic quinuclidine nucleus in the structure and the distance between the diphenylcarbinol group and the nitrogen atom. in terms of antihistamine activity and duration of action, hifenadine is superior to diphenhydramine. reduces the toxic effect of histamine, in particular, eliminates or weakens its bronchoconstrictor effect and spasmodic effect on intestinal smooth muscles, has a moderate anti-serotonin and weak anticholinergic effect, pronounced antipruritic and desensitizing properties. Khifenadine reduces the severity of the hypotensive effect of histamine and its effect on capillary permeability, does not directly affect cardiac activity and blood pressure, does not have a protective effect in aconite arrhythmias.
Hifenadine does not have a depressing effect on the central nervous system, but with individual hypersensitivity, a weak sedative effect is possible. The preparation has low lipophilicity, so its content in the brain tissues is low (0.05%), which explains the lack of a depressing effect on the central nervous system.
Pharmacokinetics. Hifenadine is rapidly absorbed in the digestive tract and after 30 minutes the preparation is determined in the tissues of the body. Cmax in blood plasma is reached in 1 hour. Metabolites and unchanged part of hifenadine are mainly excreted in urine, bile and excreted by the lungs within 48 hours. Phencarol is metabolized in the liver.

Indications

Pollinosis, food and preparation allergies, other allergic diseases, acute and chronic urticaria, edema (angioedema) angioedema, hay fever, allergic rhinopathy, dermatoses (eczema, psoriasis, neurodermatitis, pruritus), as well as infectious-allergic reactions with bronchospastic

Application

Phencarol is taken orally immediately after a meal.
Tablets 10 mg. Children aged 3–7 years - 10 mg 2 times a day (the daily dose should not exceed 20 mg); children aged 7-12 years - 10 mg 2-3 times a day (the daily dose should not exceed 50 mg), children over 12 years old - 25 mg 2-3 times a day (the daily dose should not exceed 100 mg).
For adults and children over the age of 12, it is advisable to use the preparation in a different dosage form (Phencarol , 25 mg tablets)
The duration of the course of treatment is 10-15 days. If necessary, repeat the course of treatment.
Tablets 25 mg. Single dose for adults - 25-50 mg 2-4 times a day. With pollinosis, a daily dose of 75 mg is ineffective. The maximum daily dose is 200 mg. The duration of the course of treatment is 10–20 days.
Children over the age of 12 years - 25 mg 2-3 times a day. The duration of the course of treatment is 10–20 days.
If necessary, repeat the course of treatment.
Tablets 50 mg. Adults - 50 mg 2-4 times a day. The maximum daily dose is 200 mg. The duration of the course of treatment is 10–20 days. Repeat the course of treatment if necessary.
If the next dose was not taken on time, the course of treatment should be continued using the previously prescribed doses. If necessary, you should consult a doctor.

Contraindications

Hypersensitivity to hifenadine or excipients of the preparation.

Side effects

From the nervous system: dizziness, headache.
Sometimes a weak sedative effect is possible, which manifests itself in the form of weakness, drowsiness, and a slowdown in the body's responses.
From the gastrointestinal tract: dryness of the mucous membranes of the oral cavity, dyspeptic symptoms (nausea, vomiting, bitterness in the mouth), which usually disappear with a dose reduction or preparation withdrawal.
From the respiratory system, chest and mediastinal organs: sneezing, shortness of breath.
From the side of the psyche: anxiety.
From the kidneys and urinary tract: proteinuria, interstitial nephritis.

In individuals with gastrointestinal diseases, the possibility of side effects increases.

Special instructions

Care should be taken in severe diseases of the cardiovascular system, gastrointestinal tract or liver. the preparation contains sucrose, which should be taken into account in patients with diabetes mellitus. patients with rare hereditary diseases such as fructose intolerance or sucrase-isomaltase deficiency should not take the preparation.
Use during pregnancy and lactation. It is contraindicated to prescribe the preparation in the first trimester of pregnancy. Its use is not recommended in the II and III trimester of pregnancy. There is no data on the penetration of the preparation into breast milk, therefore, the use of the preparation Phencarol during breastfeeding is contraindicated.
Children. 10 mg tablets are used in children from 3 years of age, 25 mg tablets - from 12 years old, 50 mg tablets - there is no experience of use in children.
The ability to influence the reaction rate when driving or working with other mechanisms. Persons whose work requires a quick physical or mental reaction (vehicle drivers, etc.) should first set their individual sensitivity (by short-term appointment) for sedation. These individuals need to be especially careful.

Interactions

Phencarol does not enhance the inhibitory effect of alcohol and hypnotics on the central nervous system. fencarol has weak m-anticholinergic properties, therefore, by reducing the motility of the gastrointestinal tract, it is able to enhance the absorption of slowly absorbed preparations (for example, indirect anticoagulants of the coumarin series).

Overdose

A daily dose of up to 300 mg / day does not cause severe side effects. in a high dose can cause dryness of the mucous membrane, headache, pain in the epigastric region, vomiting and other dyspeptic phenomena. there is no specific antidote. If necessary, symptomatic treatment is carried out.
