Product description

Telmista tablets are used for the following indications:

• hypertension: treatment of essential hypertension in adults;
• prevention of cardiovascular disease: reducing the incidence of cardiovascular disease in patients with:
• overt atherothrombotic cardiovascular diseases (coronary heart disease, stroke, or a history of peripheral arterial disease);
• type II diabetes mellitus with documented target organ damage.

Composition

The active ingredient is telmisartan (one tablet contains 80 mg of telmisartan).

Excipients: povidone, meglumine, sodium hydroxide, lactose monohydrate, sorbitol (E 420), magnesium stearate.

Contraindications

• hypersensitivity to the components of the preparation;
• pregnant women or women planning to become pregnant;
• obstructive biliary disorders;
• severe liver dysfunction;
• the simultaneous use of telmisartan and aliskiren in patients with diabetes mellitus or impaired renal function (GFR <60 ml / minute / 1.73 m2) is contraindicated.

Mode of application

The preparation  "Telmista" is taken orally once a day with a sufficient amount of liquid, with or without food.

Arterial hypertension treatment

The usual effective dose is 40 mg per day. For some patients, a daily dose of 20 mg may be sufficient. If the desired blood pressure is not reached, the dose of telmisartan can be increased to 80 mg once a day. Alternatively, telmisartan can be given in combination with a thiazide diuretic such as hydrochlorothiazide, which has been shown to further lower blood pressure when used with telmisartan. When considering increasing the dose, it should be borne in mind that the maximum antihypertensive effect is generally achieved after 4-8 weeks from the start of treatment.

Prevention of cardiovascular disease

The recommended dose is 80 mg once a day. The efficacy of telmisartan in doses less than 80 mg in the prevention of cardiovascular disease is unknown.

When starting treatment with telmisartan to prevent cardiovascular disease, it is recommended to monitor blood pressure and, if necessary, adjust the dose of preparations that lower blood pressure.

Application features

Children

Telmista is not recommended for use in children under the age of 18 due to insufficient data on safety and efficacy.

Drivers

When driving a car and mechanisms, it is necessary to take into account the possibility of dizziness or hypersomnia during antihypertensive therapy, including Telmista.

Overdose

Only limited information is available on human overdose.

Symptoms

The most significant manifestations of telmisartan overdose were arterial hypotension and tachycardia; there have also been cases of bradycardia, dizziness, increased serum creatinine and acute renal failure.

Treatment

Telmisartan is not removed by hemodialysis. In case of an overdose, the patient should be closely monitored and symptomatic and supportive therapy prescribed. The choice of treatment depends on the time that has elapsed after administration and on the severity of the symptoms. Consideration should be given to measures such as inducing vomiting and / or gastric lavage. Activated charcoal can be helpful in treating overdose. Serum electrolytes and creatinine levels should be monitored frequently. When arterial hypotension occurs, the patient should be placed in a supine position and quickly introduce salt substitutes and restore the volume of fluid.

Side effects

Serious side effects, including anaphylactic reactions and angioedema, are rare (≥ 1/10000 to <1/1000), and acute renal failure has also been observed.

Storage conditions

Store in its original packaging to protect from light at a temperature not exceeding 30 ° C, out of the reach of children.
