Pharmacodynamics. α-lipoic acid - a substance that is synthesized in the body and plays the role of a coenzyme in the oxidative decarboxylation of α-keto acids; plays an important role in the formation of energy in the cell. helps to lower blood glucose levels and increase the amount of glycogen in the liver. deficiency or impairment of the metabolism of α-lipoic acid due to intoxication or excessive accumulation of certain breakdown products (for example, ketone bodies) leads to a violation of aerobic glycolysis. α-lipoic acid can exist in two physiologically active forms (oxidized and reductive), which are characterized by antitoxic and antioxidant effects. α-lipoic acid affects the metabolism of CS, takes part in the regulation of lipid and carbohydrate metabolism, improves liver function due to hepatoprotective, antioxidant and detoxification effects. in terms of pharmacological properties, α-lipoic acid is similar to vitamins of group B.

Pharmacokinetics. After oral administration, α-lipoic acid is rapidly and almost completely absorbed in the digestive tract. α-Lipoic acid is significantly converted during its primary passage through the liver. Significant individual fluctuations in the systemic availability of α-lipoic acid are noted. Excreted by the kidneys, mainly in the form of metabolites.

Application

Thiogamma tablets. the preparation is prescribed for adults. tablets are taken orally whole with plenty of water. daily dose - 1 tablet of thiogamma preparation (which corresponds to 600 mg of α-lipoic acid), which must be taken as a single dose approximately 30 minutes before the first meal. the duration of treatment is 1–4 months.

In cases of severe manifestations of the disease, it is advisable to start treatment with parenteral administration of Tiogamma Turbo for infusion. In the future, you should continue taking the preparation Tiogamma in tablets at a dose of 600 mg / day. Simultaneous food intake may interfere with the absorption of the preparation.

Tiogamma Turbo. The preparation is administered directly from the bottle (that is, without solvent) as an intravenous drip infusion at a dose of 600 mg / day (contents of 1 bottle) for at least 30 minutes.

Due to the fact that α-lipoic acid is sensitive to light, the vials should be stored in cardboard packaging until they are used directly.

At the beginning of the course of treatment, the preparation Thiogamma Turbo is administered intravenously. The course of treatment is 2–4 weeks.

For further therapy, oral forms of thioctic acid preparations are used at a dose of 600 mg / day.

Contraindications

Thiogamma tablets

Side effects

From the side of the blood system: hemorrhagic rash (purpura), thrombophlebitis.

Allergic reactions: urticaria or eczema at the injection site, systemic allergic reactions that can lead to the development of anaphylactic shock.

Others: decreased blood glucose, hypoglycemia, dizziness, hot flashes, increased sweating, headache, blurred vision, nausea, vomiting, heart pain, tachycardia. After a quick injection of the preparation, a feeling of compression in the head, respiratory depression is possible, but these phenomena spontaneously disappear.

The main factor in the effective treatment of diabetic polyneuropathy is the optimal correction of the patient's blood glucose level.

With parenteral use of the preparation Thiogamma Turbo, there is a risk of allergic reactions, including anaphylactic shock, therefore, patients should be checked for such reactions. In case of the appearance of such signs as itching, nausea, malaise, the administration of the preparation should be stopped immediately and the necessary therapeutic measures should be taken.

Some patients with decompensated or inadequately controlled diabetes and a deterioration in general health may develop severe anaphylactic reactions associated with the use of Thiogamma Turbo.

When treating patients with diabetes mellitus, especially at the beginning, frequent monitoring of blood glucose levels is necessary. In some cases, it is necessary to adjust the dose of antidiabetic preparations to prevent hypoglycemia.

During the treatment of polyneuropathy, due to regeneration processes, a short-term increase in sensitivity is possible, accompanied by paresthesia with a feeling of "creeping".

A certain limitation for the administration of thioctic acid preparations is elderly age (over 75 years).

Warning: regular alcohol consumption is a significant risk factor for the development and progression of the clinical picture of neuropathy and can thus negatively affect the treatment process. Therefore, patients with diabetic polyneuropathy are usually advised to abstain from alcohol if possible. Alcohol restriction also applies to breaks between courses of treatment.

Use during pregnancy and lactation. There is insufficient data on the use of the preparation, therefore, its appointment during pregnancy and lactation is not recommended.

Children. The efficacy and safety of the preparation in children have not been established, therefore it should not be prescribed to patients of this age category.

The ability to influence the reaction rate when driving vehicles and other mechanisms. There are no particular caveats.

Interactions

Thiogamma tablets. α-lipoic acid reacts with ionic metal complexes (for example, with cisplatin), so thiogama can reduce the effect of cisplatin. α-lipoic acid forms poorly soluble complex compounds with sugar molecules.

α-Lipoic acid is a metal chelator, therefore, it should not be used simultaneously with metal compounds (for example, with preparations containing iron or magnesium, dairy products, since they contain calcium).

If the total daily dose of Tiogam is taken 30 minutes before breakfast, iron and magnesium supplements should be used during lunch or in the evening.

With the simultaneous use of α-lipoic acid enhances the action of insulin and oral hypoglycemic agents.

Tiogamma Turbo. α-Lipoic acid reacts with ionic metal complexes (for example with cisplatin), therefore Thiogamma Turbo can reduce the effect of cisplatin. With glucose molecules, α-lipoic acid forms poorly soluble complex compounds. Therefore, the solution of α-lipoic acid is incompatible with the solution of glucose, Ringer and solutions, which can react with SH-groups or disulfide compounds.

Overdose

The clinical toxicological profile may initially cause psychomotor agitation or dizziness and is generally associated with seizures such as generalized seizures and lactic acidosis. There have been reports of such intoxicating effects with the use of high doses of α-lipoic acid, such as hypoglycemia, shock, acute skeletal muscle necrosis, hemolysis, DIC (DIC), inhibition of brain function and disorders of the internal organs.

Therapeutic measures in case of intoxication. If severe intoxication with α-lipoic acid is suspected, the patient should be immediately hospitalized in a hospital and general therapeutic measures provided for in case of poisoning should be started (in particular, stimulation of vomiting, gastric lavage, the use of activated charcoal, etc.). Treatment of generalized epileptic seizures, lactic acidosis and other life-threatening consequences of intoxication should be carried out in accordance with modern principles of intensive treatment and depending on the symptoms.

The use of hemodialysis, hemoperfusion and filtration techniques for the accelerated elimination of α-lipoic acid has not yet proven convincing advantages.
