Pharmacological properties

Pharmacodynamics. Gyurza poison (venenum macroviperae lebetina), containing phospholipase, phosphodiesterase, hyaluronidase, as well as other active substances, is the key active ingredient of vipratox liniment. the pharmacological action of the vipratox liniment is caused by the toxins that make up the venom of the gyurza. postsynaptic neurotoxins provide a specific blockade of n-cholinergic receptors of the subsynaptic membrane of skeletal muscles and certain parts of the central nervous system. presynaptic nerve endings are affected by presynaptic neurotoxins, which contribute to an increased release of neurotransmitters. actively affecting cell membranes, membrane-active neurotoxins (cardiotoxins, cytoxins, which are polypeptides in their chemical structure) also cause their depolarization.
Along with the specific effect, which directly depends on the composition of the poison, the therapeutic effect of the preparation is caused by reflex reactions, the occurrence of which is associated with irritation of skin receptors, the effect of biogenic substances formed during the local effect of the preparation on tissues, the effect on the reactions of the body's immune system and stimulation of the hypothalamic-pituitary -adrenal axis. When applied locally, the poison of gyurza has an irritating and analgesic effect.
Salicylic acid, when applied externally, has an antiseptic, distracting, irritating and keratolytic effect, contributing to the optimal penetration of the active ingredients of the ointment.
Due to its excellent lipophilicity, fir oil easily penetrates the epidermis, irritates the endings of the sensory nerves of both superficial vessels and deeper tissues.
Due to the inclusion in the composition of camphor, the preparation has a local irritating, analgesic and anti-inflammatory effect and, by stimulating the sensitive nerve endings of the skin, improves tissue trophism.
Pharmacokinetics. Has not been studied.

Indications

Symptomatic therapy of pain in rheumatism, neuralgia, sciatica, lumbago, radiculitis, myalgia, arthralgia.

Application

To avoid side effects, you should first apply a small amount of liniment to the skin in order to determine the individual sensitivity to the preparation.
Depending on the surface area of ​​the painful area, liniment should be applied in a small amount - 5–10 g and rubbed into the skin thoroughly 1–2 r / day until the pain syndrome stops.
The duration of the course of therapy is determined by the doctor depending on the severity of the disease and the effectiveness of therapy.
Children. There is no experience of use in children, therefore, the use of the preparation in this category of patients is not recommended.

Contraindications

Hypersensitivity to the components of the preparation. Whooping cough, a tendency to bronchospasm and seizures, acute pulmonary tuberculosis, impaired cerebral and coronary circulation, a tendency to vasospasm, severe renal and liver dysfunctions, pustular skin diseases, damage to the integrity of the skin at the site of application of the preparation, fever, general fatigue.

Side effects

Local allergic reactions are possible (including itching, burning, rash, dryness, peeling, irritation, contact dermatitis, urticaria), fever.

Special instructions

Do not apply to skin with manifestations of an allergic reaction.
Avoid contact with eyes, open wounds and mucous membranes. In case of accidental contact with eyes or mucous membranes, rinse thoroughly with plenty of running water.
Use with caution in the elderly.
Use during pregnancy and lactation. Contraindicated during pregnancy and lactation.
The ability to influence the reaction rate when driving or working with other mechanisms. The preparation does not affect the reaction rate when driving or operating other mechanisms.

Interactions

Have not been studied. A sufficient number of controlled studies of preparation interactions have not been conducted, it is possible that excessive use of salicylates for topical use may increase the effect of coumarin anticoagulants and antiplatelet preparations. Caution should be exercised in patients taking coumarin anticoagulants and antiplatelet preparations, including acetylsalicylic acid.

Overdose

In case of accidental contact with mucous membranes, the ointment causes severe irritation. allergic reactions are possible (itching, dermatitis, peeling of the skin). you should stop using liniment and wash off its remnants from the skin. if necessary, symptomatic therapy is indicated.
Improper use. Accidental swallowing of the ointment can cause digestive symptoms such as vomiting and diarrhea. Therapy is symptomatic.
If a large amount of the ointment is accidentally swallowed, symptoms of acute poisoning such as nausea, vomiting, abdominal pain, headache, dizziness, a feeling of heat / redness, convulsions, respiratory depression and coma may appear.
Therapy for patients with severe gastrointestinal or neurological symptoms of poisoning is symptomatic. Do not induce vomiting.
