Pharmacological properties

Pharmacodynamics. Zalain / Zalain ovuli is an antifungal preparation, an imidazole derivative with high fungicidal activity, intended for topical use in gynecology. The mechanism of action is to inhibit the synthesis of ergosterol and increase the permeability of the cell membrane, which leads to the destruction of pathogens. Effective against pathogenic yeast fungi (candida albicans, candida spp. and malassezia furfur), dermatophytes (trichophyton, epidermophyton and microsporium spp.) and pathogens that cause infectious diseases of the skin and mucous membranes, including gram-positive strains (staphylococcus), streptococcus.

Pharmacokinetics. There is no systemic absorption.

Indications

Zalain cream. local treatment of fungal skin infection caused by dermatophytes and yeasts: epidermophytosis of the feet, inguinal epidermophytosis, dermatomycosis of smooth skin, trichophytosis of the beard and mustache area, ringworm of the hands, candidiasis, pityriasis versicolor.

Zalain ovuli, vaginal suppositories. Local treatment of vaginal candidiasis.

Application

Cream. the cream is applied 1 or 2 times a day (preferably at night or in the morning and in the evening) in a thin layer, gently and evenly on the affected area of ​​the skin, capturing about 1 cm of healthy skin around it.

The duration of treatment depends on the etiology of the pathogen and the location of the infection. Typically, use is recommended for about 4 weeks to achieve clinical remission with a negative microbiological follow-up to prevent relapse, but in many cases, clinical recovery occurs earlier - between 2 and 4 weeks.

Pessaries. Adult women: 1 pessary is inserted deep into the vagina in the evening before going to bed, 1 time per day. If the clinical signs of the disease persist, it is possible to reapply the preparation after 7 days.

Contraindications

Cream: hypersensitivity to any antifungal derivatives of the imidazole group in history or to any other component of the preparation.

Pessaries: hypersensitivity to antifungal preparations, imidazole derivatives or any excipients of the preparation; simultaneous use of the preparation with latex condoms or pessary is contraindicated.

Side effects

Cream. the preparation showed good tolerance when applied to both intact and damaged skin.

Contact dermatitis (dry skin, burning, itching, urticaria) may appear locally. Possible hypersensitivity reactions, local transient erythematous reactions that do not require discontinuation of the preparation.

The medicinal product contains methyl parahydroxybenzoate, which may cause allergic reactions (possibly delayed).

The preparation contains sorbic acid, which can cause local skin reactions (for example, contact dermatitis).

Pessaries. Sometimes a transient local irritation reaction (burning sensation and itching) may occur. This may be a consequence of the action of cell fragments during the death of pathogens, therefore, such a reaction is most likely a sign of successful therapy. Allergic reactions often occur.

Special instructions

During treatment, it is not recommended to use soap with an acidic ph, you should use mainly cotton linen, do not douching.

It is also advisable to apply Zalain antifungal cream to the vulva and perineum.

When using the preparation, you should refrain from sexual intercourse. It is recommended to consider the possibility of simultaneous treatment of the sexual partner.

Therapy can be performed during menstruation.

It is necessary to identify and eliminate factors (hygiene or lifestyle) that contribute to the development and manifestations of fungal infection. It is also recommended to treat other pathogens that may be associated with candidiasis.

The therapy should be discontinued if a local allergic reaction occurs.

In the absence of characteristic clinical signs of vaginal candidiasis, a positive microbiological test result in itself is not an indication for treatment.

Zalain cream cannot be used in ophthalmology. Acidic detergents should be used with caution in patients, since the multiplication of Candida spp. Increases in an acidic environment.

Application during pregnancy or lactation. There are no data on the presence of embryotoxicity and teratogenic effects of sertaconazole. Given the method of administration (single dose of treatment) and the absence of systemic absorption, it is possible to use sertaconazole for pregnant women only in cases where the expected benefit to the mother outweighs the potential risk to the fetus.

There is also no data on the penetration of sertoconazole into breast milk. Therefore, during breastfeeding, the preparation should not be used, unless, in the opinion of the doctor, the expected benefit to the mother outweighs the potential risk to the child.

After external use of the preparation in large quantities, plasma levels were not observed. Since the harmful effects of the preparation on newborns or pregnant women have not been investigated, before prescribing Zalain during pregnancy and lactation, the benefit-risk ratio for the woman and the fetus / child should be assessed.

Children. Do not use in children.

The ability to influence the reaction rate when driving or working with other mechanisms. Does not affect.

Interactions

For the preparation in the form of a cream, interactions have not been established. with the simultaneous use of pessaries with local contraceptives, a decrease in their spermicidal effect may be noted. do not recommend the simultaneous use with latex condoms or pessary due to the risk of damage.

Overdose

When using the cream, an overdose is unlikely. after vaginal use of pessaries, overdose was practically not observed.

Storage conditions
