Pharmacological properties

Pharmacodynamics. Betamethasone dipropionate is a synthetic fluorinated corticosteroid that has anti-inflammatory, anti-allergic and vasoconstrictor effects. when applied topically, salicylic acid has a keratolytic effect.
Pharmacokinetics. Absorption in the body of betamethasone dipropionate is possible mainly after long-term treatment on a large surface of the skin.

Indications

For local treatment of dermatoses sensitive to corticosteroids, such as chronic, erythematous or hyperkeratosis psoriasis and other dermatoses of an erythematous-squamous nature, such as seborrheic dermatitis (eczema), dry eczema in the desquamative phase, lichenification.

Application

Belosalik ointment is applied only externally 2 times a day by applying a thin layer to the affected surface and rubbing it lightly. the duration of treatment is usually limited to 3 weeks. the frequency and duration of use, different from the recommended one, can be determined by the doctor, focusing on the severity of the disease. in cases of a mild course of the disease, a single application throughout the day is sufficient.
The maximum daily dose should be gradually reduced to as low as possible to control symptoms. The preparation is used in children aged 2 years and older. The duration of treatment in children aged 2 years and older should be limited to 5 days.

Contraindications

Hypersensitivity to betamethasone, salicylic acid or any of the excipients of the preparation. as well as rosacea, acne, common plaque psoriasis, perianal and genital pruritus, diaper dermatitis, perioral dermatitis, rosacea, skin manifestations of syphilis, skin tuberculosis, other bacterial and fungal skin infections without proper antibacterial and antifungal therapy, molluscum contagiosum, dermatomal reactions after vaccination, varicose veins, viral infections (eg herpes simplex, shingles, chickenpox). should not be used under occlusive dressings.

Side effects

When using local corticosteroids, the following side reactions may occur: burning, itching, irritation, dry skin, tingling of the skin, skin tightening, cracking of the skin, a feeling of warmth, lamellar peeling of the skin, focal skin peeling, erythema, folliculitis, hypertrichosis, acne, hypopigmentation, perioral dermatitis and allergic contact dermatitis.
The following adverse reactions may occur more frequently with occlusive dressings: skin maceration, secondary infection, skin atrophy, stretch marks and prickly heat.
In persons with individual intolerance to the components of the preparation, hypersensitivity reactions are possible.
Stretch marks and vasodilation, mainly on the face, can be the result of prolonged continuous application of the preparation.
Any side effects that occur with the systemic use of glucocorticoids, including suppression of the adrenal cortex, can occur with their topical application.
With local application of salicylic acid, such changes in the skin may occur: dryness, peeling, irritation, contact dermatitis, allergic reactions (urticaria, itching), which require discontinuation of the preparation.
With prolonged use, the preparation may be absorbed into the general bloodstream and the development of side effects characteristic of salicylates: tinnitus, dizziness, epigastric pain, nausea, vomiting, acidosis, rapid breathing.

Special instructions

The preparation is not intended for use in ophthalmology. avoid contact with eyes, mucous membranes, wound surfaces and ulcers.
When applied to the skin of the face, the duration of the course of therapy should be limited to 5 days.
If skin irritation or hypersensitivity occurs during the use of the preparation, treatment should be discontinued. In the presence of infection, appropriate therapy should be prescribed.
If dandruff or keratinization disappears, continue treatment with only corticosteroids. Any side effects that occur with the use of systemic corticosteroids, including inhibition of the function of the adrenal cortex, can also occur with local application of GCS, especially in children.
Systemic absorption of corticosteroids or salicylic acid when applied topically will be higher if treatment is carried out on large surfaces of the body or when using occlusive dressings. Appropriate measures should be taken in such cases, especially when treating children.
Long-term preparation  therapy should be avoided in all patients, regardless of age.
With the development of excessive dryness or increased skin irritation, the preparation should be discontinued.
Topical corticosteroids can cause psoriasis for a variety of reasons, including recurrence of symptoms followed by the development of tolerance, the risk of pustular psoriasis and local systemic toxicity due to a decrease in the protective function of the skin. Patients with impaired liver function are most sensitive to systemic action. Careful observation of the patient is necessary. Topical corticosteroids can distort the clinical picture.
Relapse is possible if treatment is interrupted. An exacerbation of the infection may occur, and healing may also be slowed down.
The preparation should not be applied to mucous membranes or areas around the eyes due to the keratolytic action of salicylic acid.
Application of the preparation to areas with atrophied skin is contraindicated.
Use during pregnancy and lactation. Pregnancy. Since the safety of the use of topical corticosteroids in pregnant women has not been established, the preparation should not be used in the first trimeter of pregnancy. Prescribing corticosteroids later in pregnancy is possible if the expected benefit to the mother clearly outweighs the potential threat to the fetus. Preparations in this group are contraindicated for pregnant women in high doses and for a long time.
Breastfeeding period. At present, it is not clear whether corticosteroids, when applied topically as a result of systemic absorption, can penetrate into breast milk, therefore, when deciding whether to stop breastfeeding or discontinue the preparation, the need to prescribe the preparation should be taken into account.
Children. The preparation is used in children aged 2 years and older.
Application in children is possible only after consultation and under medical supervision. Consideration should be given to the greater absorption of the preparation in children due to the greater value of the ratio of the skin surface area to the child's body weight, which may manifest itself as an increase in systemic toxicity and inhibition of the hypothalamic-pituitary-adrenal system. The duration of treatment in children should be kept as short as possible. Children are more likely to have signs of suppression of the hypothalamic-pituitary-adrenal system and external corticosteroid effects under the influence of local corticosteroids than in adult patients. In children receiving treatment with topical corticosteroids, suppression of the hypothalamic-pituitary-adrenal system, Cushing's syndrome, linear growth retardation, insufficient body weight gain, and increased intracranial pressure may be observed. Manifestations of adrenal suppression in children: low blood plasma cortisol levels and lack of response to ACTH stimulation. Increased intracranial pressure is manifested by protrusion of the fontanelle, headache, bilateral edema of the optic nerve head.
Do not use under diapers.

Interactions

Topical application of salicylic acid should not be combined with oral administration of preparations containing acetylsalicylic acid and other NSAIDs. do not use in conjunction with benzoyl peroxide and topical retinoids. Salicylic acid can increase the permeability of the skin to other topical preparations and thereby increase their absorption. in addition, salicylic acid can enhance the undesirable effect of methotrexate and the hypoglycemic effect of oral antidiabetic preparations - sulfonylurea derivatives.

Overdose

With prolonged or excessive use of local corticosteroids, it is possible to suppress the pituitary-adrenal function with the development of secondary adrenal insufficiency and the appearance of symptoms of hypercortisolism, including Cushing's disease. Excessive or prolonged use of topical salicylic acid preparations can lead to symptoms of salicism.
Treatment. Prescribe appropriate symptomatic therapy. Symptoms of acute hypercortisolism are usually reversible. If necessary, the electrolyte balance should be corrected. In the case of chronic toxic effects, a gradual withdrawal of corticosteroids is recommended.
Treatment of salicism is symptomatic. Measures are taken to remove salicylates from the body more quickly. In case of overgrowth of resistant microorganisms, it is recommended to discontinue treatment with the preparation and prescribe the necessary therapy. Orally administer sodium bicarbonate to alkalinize urine and increase urine output.
